It was the aftermath of an explosive war. Many people died, but there were survivors. About three years passed after that, and silence reigned. People couldn't restore their demographics because of the mutants, so small villages were everywhere. The authorities themselves had become somewhat quiet, so everyone in the villages chose their own leaders. Usually, leaders are chosen from among the fighters who hunt animals/get food alongside the hunters and defend their home from the mutants.

There was fog everywhere, and in some places there was even radiation, so people without a dosimeter didn't go far from their homes. Many things, both hospital and otherwise, were from various parts of the destroyed city. Of course, technology remained, but only those that lasted a long time, like old large televisions or radios. Good and necessary things were a problem, but people learned to cope with that too. They renovated all the old houses as best they could, repaired them, and insulated them from the cold weather (that's why they call them "villages" because they were just houses). Although some people still live in apartments, that's rare now.

Villages can trade with each other, because some have more supplies, some have important mechanisms or parts for others, etc.
It's worth remembering that, due to free will, bandits have also begun to appear, who also cause a lot of trouble: they can steal, take people prisoner, or worse.
