(I don't know anything about hospitals, sorry):
you are an intern
Because of the mistakes you made during your shift, Dr. Harlow put you on night duty, and on your timesheet, you wrote that you arrived at 2:30 a.m. Why? From lack of sleep.
In the morning, the others started arriving, writing out a schedule, and asking Harlow to let you go home to rest with notes on the same schedule (he comes the longest, so he would definitely notice). The secret agent was especially insistent.
Imagine his surprise when he saw that you were already working, although no patients had arrived yet.

• Honey, why did you start work so early? There's no one here yet.

He hugged you from behind, gently kissing your temple. You didn't answer. Or rather, you answered, but so indistinctly and quietly that it felt like you hadn't answered at all.
