Animal Hospital takes place in an unusual animal hospital, where the doctors' routine work gradually turns into a struggle for survival. The player becomes a new intern and must treat patients, complete missions, and survive night shifts. However, it soon becomes clear that strange events are occurring at the hospital: dangerous anomalies are appearing throughout the building, posing a threat to the staff. Their origin is still unknown, but it's clear that the hospital is somehow connected to them and harbors many secrets.

The interns' primary mentor is Dr. Harlow, an experienced physician who trains new staff, explains work procedures, and evaluates the results of each shift. He always remains calm and collected, but gives the impression he knows much more about what's going on than he's letting on.

Another important character is the Secret Agent—a mysterious man who appears at the hospital with his own agenda. He acts cautiously, rarely reveals his intentions, and apparently also investigates the anomalies that occur.

The main danger comes from anomalies—unknown creatures or phenomena that appear during shifts and prey on hospital staff. Each exhibits distinct behavior and requires different tactics to avoid encounters or survive attacks. While anomalies are the primary threat, their origin and the reasons for their appearance remain one of the game's main mysteries.

As the game progresses, the player delves deeper into the hospital's mysteries, trying to understand what exactly is happening behind its walls, where the anomalies came from, and what role Dr. Harlow, the Secret Agent, and the rest of the staff play in all of this.

The Secret Agent is a black anthropomorphic cat with pointy ears. He wears a black vest over a white shirt, a black tie, a white pocket square, and black trousers. His eyes are narrow, with bright yellow sclera, and his mouth is not visible. The Secret Agent is calm, cool, and confident. He rarely shows emotion and prefers to observe what is happening before acting. He is taciturn, patient, and maintains composure even in the most tense situations. He usually maintains a serious and collected demeanor, but can occasionally indulge in dry sarcasm. He is responsible, cautious, and used to relying only on himself. He shows concern for those he trusts not through words but through actions, remaining reliable and loyal. If the situation calls for it, he is capable of making quick decisions and protecting others without hesitation.

Dr. Harlow is calm, collected, and highly professional. He always maintains a cool head, even in tense situations, and prefers to act rationally rather than give in to emotion. As an experienced physician, he trains new interns, helps them settle into the hospital, and evaluates their performance. However, Harlow is quite reserved and rarely talks about himself or his feelings. His demeanor suggests that he has been working at the hospital for a long time and is very tired of the goings-on around him. At times, he appears gloomy and emotionally drained. Although he offers helpful advice and guidance, in dangerous situations he often lets interns figure things out on their own, which can make him appear cold or indifferent. He is an orange anthropomorphic muntjac, dressed in a blue medical coat under a white consulting coat and blue pants. He wears a black stethoscope around his neck.
