"Eyes from Above" - Mass of Eyes
What it looks like: A cluster of eyeballs and chunks of flesh hanging from the ceiling in patient rooms or in a CCTV room. It looks creepy and pulsating.
Behavior: When you enter a room, the music turns off and the message "KEEP YOUR EYES UP" appears at the bottom of the screen. Looking up causes you to lose 2-5 sanity points every half second.
How to neutralize: You can use eye drops to calm the anomaly, but this will still cost you some Sanity. It's often easier to simply avoid looking up and quickly leave.
Bed Monster
What it looks like: a black figure with round red eyes and long arms, with black smoke billowing around it. It hides under patients' beds.
Where it appears: Under the bunks, next to a transparent red box on the floor. If you step on it, the monster will grab you.
What it does: When grabbed, you lose 10 Sanity immediately, then 5 Sanity every 2 seconds until you break free. It can also attack patients, which will trigger an emergency.
How to avoid/neutralize:
Don't step on the red boxes.
If the monster has already grabbed you, you can try to distract it with maple syrup: approach the box with syrup in your hand, and the monster will switch to it.
You can also use a stun gun or a gun, but this is risky.
Stalker
What it looks like: An elongated black silhouette, resembling a rabbit, with an unnaturally wide, toothy smile.
Where found: appears unexpectedly around corners or in dark corridors.
What it does: If you look at it directly, you lose 10 Sanity for each glance. It doesn't rush at you right away, but it keeps an eye on you.

Slimbert
Slimebert is an enemy in the Animal Hospital. It appears during Slime Shift and has a chance to appear on Shift 10 and above.[1] Appearance
Slimebert is a gigantic creature composed entirely of bright neon-green slime, with a soft, gelatinous body that seems to constantly spread and drip. It has two large purple eyes on jagged stalks growing from the top of its head, giving it a strange, alien appearance. Its face is dominated by a huge, gaping black mouth that extends deep into its body, creating the impression of emptiness. Its rounded, teardrop-like form has no clearly defined limbs, instead appearing as dense masses of slime, and when it moves, bubbles and globs of slime form on its body. Behavior
When Slime Shift begins, Slimbert approaches the reception window and says, "I can't wait any longer, I'm hungry." He then walks past the reception desk and begins wandering around the Animal Hospital. Slimbert eats any players who get too close, causing them to lose 20 Sanity immediately and an additional 3 Sanity per second while being digested. He can also eat patients, causing them to pass out. While he can eat security staff, this will have no effect on them. Slimbert will leave once Slime Shift ends and will not appear again after the first Slime Shift. Strategies
It's recommended to always avoid Slimebert. This is easy to do, as Slimebert only moves in a straight line and is much slower than the player.
If you have cough syrup, Slimebert will not attack you; instead, the following text will appear: "Slimbert choked on your cough syrup!"
A fire extinguisher can temporarily stun it, but it won't kill it.
A tiny anomaly
This anomaly appears very small, has a very high-pitched voice, and may have other abnormal features.
Currently, the only patient with this anomaly is a pink rabbit with three glowing red eyes.
Main Monsters/Anomalies 2
"Hares that come out of the wall" - Hiders
You probably mean them: they're flat, like they were cut out of paper, with eyes and that creepy smile, and they appear right out of the walls.
What they look like: Thin, almost two-dimensional creatures, their color matched to the wall they sit on. Only their faces and a slight outline of their bodies are visible.
Where they appear: in patients' rooms and hallways, often in groups of 1-6. They emit heavy breathing if you get close. What they do: If you get close and stare at them, they jump off the wall, turn black, and begin chasing you for 5-6 seconds. When attacking, they drain 10 Sanity and can steal an item from your hand.
How to fight: A fire extinguisher is the most reliable way: spray them and they will disappear.
A stun gun or a firearm also work.
You can simply run away: in a few seconds they will dissolve into the black fog.
A neat trick: lure them to the Bed Monster - it will grab Hider, and you will get rid of two threats at once.

Rooms 1–5 (Medical Wing)
These are standard wards for receiving and treating patients. There are no mini-games, but this is where the most common dangers arise:
There may be a Bed Monster hiding under the beds.
Hiders ("hares") can sit on the walls.
Cabinets and nightstands usually contain consumables: bandages, pills, ointments, etc. Rooms 1–5 are for quick navigation: if Dr. Harlow says "open access to rooms 1–3," that means you can go in and treat patients, but the doors may be locked until then. Rooms 6–8 (Emergency Wing)
This is no longer just an examination, but special procedures with mini-games.
Room 6 is the X-Ray Room. Here, you must complete a minigame: memorize a sequence of four colors that light up and then repeat it. Without successfully completing this minigame, an accurate diagnosis cannot be made. Room 7 is the Heart Monitor Room. This is where critically ill patients are monitored: the monitor may show abnormal pulse rates—a clue that the patient is unusual and requires special attention.
Room 8 — Surgery Room. The most important and dangerous room. During surgery, tentacles (Surgery Tentacles) appear: you can't stun them or shoot them, as the patient will die instantly. The only way to deal with them is to calmly complete the surgery minigame, ignoring the tentacles.

Main locations
The Lobby is the entry point, the place where players gather before their shift. Starting instructions are usually given here, and you might encounter NPCs like Ron from Accounting.
Examination Rooms are where you examine patients: look at their eyes, teeth, and behavior. This is where abnormalities (such as twitching or an unnatural smile) are most likely to appear. Treatment Rooms are where animals are placed after examinations. The Bed Monster may appear here—it can be dangerous if left too long.
The Surgery Room is one of the most tense areas, featuring special enemies like the Surgery Monster. It's often accompanied by an unsettling atmosphere and sound effects. The Staff Room is the staff's break area. Here you can find useful items, notes with lore, and occasionally encounter minor characters (like Barney).
Storage / Supply Closet - a cramped space with boxes and supplies

Herbs - treat headaches. Thermometer - stabilizes temperature during fever. Medicine - helps with stomach aches. Cough Syrup - used for flu. Maple Syrup - replenishes low blood sugar. Eye Drops - treat dry eyes.
IV Drops - combat dehydration.
Ointment - helps with burns and rashes.
Bandage - stops active bleeding.
Medkit (First Aid Kit) - Treats bruises and lacerations. Important details
Don't use any item haphazardly. Each item has its own purpose. Giving the wrong one could harm the patient.
Anomalies are a special case. If you "cure" an abnormal patient incorrectly (give the wrong item), it won't remove the error penalty, but prolonged use of the wrong remedy can trigger the appearance of a Skinwalker.
The items are free. You don't need to purchase them—they appear on counters and tables near patients' rooms. After use, they respawn after a short time.

Surgery Monster - an enemy associated with the surgical area. Anomaly Patients - animals with eerie visual and behavioral anomalies (like the ones you've already seen: three-eyed, twitching, unnaturally smiling, etc.). They are not individual names, but procedurally generated cases.
Bed Monster is a monster associated with a hospital bed; it can appear in hospital rooms.
Camera Figure is a creature that appears through surveillance cameras.
What restores memory:
Coffee – from the coffee machine in the lobby or break rooms. Gives +5% per sip, for a total of 3 sips (+15% total). 3-minute cooldown.
Chocolate – Restores 20% per bite, up to 3 times (for a total of +60%). Often found in crates, on tables, or for sale.

Classes (you can choose/partners):
Intern is a free starting class. It provides a Sanity bonus at the start of the round (up to +20% at level 3). It's a good way for new players to get their bearings.
Nurse — costs only 20 Animal Coins. Increases inventory capacity (+1 slot at level 1, up to +3 at level 3). A cheap and practical option. Secretary — 120 Animal Coins. Restores sanity when registering patients and provides a small bonus to starting sanity. Helps keep sanity under control.
Paramedic — 250 Animal Coins. Starts with "Speed Cola": 6 uses at level 1, regeneration of 1 use per round at level 3. Handy for urgent tasks.
Doctor — 900 Animal Coins. Passively restores Sanity when healing patients; at higher levels, the bonus becomes more significant.
Psychologist (Psychologist) — 500 Animal Coins. Doubles all sanity effects and grants a chance to ignore sanity loss (up to 15% at level 3). Risk and reward: both benefits and drawbacks become stronger.
Surgeon — 2,500 Animal Coins. After healing a patient, gains sanity and a short-term speed boost (8 seconds at level 2). One of the most powerful free classes.
Security — 1,250 Animal Coins. Starts with an X-Taser: 5 charges at level 1, regenerates 1 charge per round at level 3. Useful for anomaly control.
Head Nurse is a Robux-based class. It immediately gives you +3 inventory slots and a Sanity bonus; at level 3, you start with all the items. Very convenient, but not free.
Secret Agent – also for Robux. Starts with a firearm (20–30 uses), at level 3 regenerates 1 use per round and gains a sanity bonus. Most effective against dangerous anomalies.

Description of Secret Agent
The Secret Agent is one of the playable classes, as well as one of the two Robux-exclusive classes in the game, along with the Secret Agent and the Head Nurse.
They can be purchased for 890 Robux (or 360 or 1000 Robux depending on the region) in the class shop in the server lobby. Selecting this class at the start of the game will grant the player a pistol with 20 ammo. The Secret Agent upgrade is especially useful if you're focusing on fighting Anomalies. However, before purchasing this class, consider whether it's worth it, as it's the most expensive class in Robux. Appearance
The Secret Agent is an all-black, cat-like creature with pointed ears. He wears a black vest over a white collared shirt, a black tie, a white pocket square, and black trousers. His face features slit-shaped eyes with bright yellow sclera, and his mouth and nose appear invisible unless behind a glass door. Interesting Facts
The Secret Agent and Head Nurse are the only two classes that can only be purchased with Robux.
The Secret Agent is the only class with eyes from the normal set that does not have eye highlighting.
Two other characters who don't have eye lighting are Barney and Ratthew.
Although their mouths are not usually visible, if you look at the Secret Agent through a glass door, their mouth and nose will be visible.
Description of the Surgeon
Surgeon is one of the playable classes in the game.
It can be purchased for 2,500 Animal Coins in the class store in the server lobby. When equipped, this class grants a sanity boost and a short-term speed boost after healing a patient.
Appearance
The Surgeon is a dark orange moose-like creature with lighter orange markings on its face. It wears a white medical uniform under a turquoise coat. A black stethoscope hangs around its neck. On its head is a blue cap with a fleece and a speculum with a brown band. It has slanted eyes with white irises, and its mouth is covered by a blue surgical mask. Two horns protrude from either side of its head, the color of which matches the markings on its face.

The Head Nurse is one of the playable classes and one of the two Robux-exclusive classes in the game, along with the Secret Agent and the Head Nurse.
They can be purchased for 190 Robux (or 240 or 80 Robux depending on the region) in the class shop in the server lobby. Selecting this class grants three additional inventory slots. With the second upgrade, in addition to the bonus you receive at level 1, the Head Nurse can gain an additional 10% sanity at the start of the game. And with the final upgrade, all items will appear in your inventory from the very beginning of the game.
The Head Nurse is a great class for solo play, especially as you level up. In a team, you'll focus on treating patients with this class, as it's incredibly useful. Appearance

Firefighter is one of 11 playable classes in the Animal Hospital.
Appearance
Fireman is a blue-gray elephant with predominantly pink ears and short tusks. He wears a black and blue hat and a black and blue uniform with yellow stripes on the sleeves and torso. His pants have two yellow stripes at the bottom and two pockets near the kneecaps. Fireman also has a small tail. Interesting Facts

This was the only new class added in this update.
The firefighters' starting item, the special fire extinguisher, charges in your hand and does not need to be brought to a charging station like a regular fire extinguisher.

They can be purchased for 500 Animal Coins in the server lobby class shop. Selecting this class grants the player double mental health effects (both positive and negative), 100% of the initial mental health, and passive mental health for patients who have undergone treatment, as well as for those in the hospital, and for those who drink coffee. The Psychologist is a green, rabbit-like creature with protruding ears, wearing a dark green shirt and brown trousers, a black tie, and brown shoes. They have large eyes with triangular black eyeliner on the left and right sides, huge round brown glasses, and a large black mustache covering their mouth.

Sam
Sam is a calico cat with a white face, with a large orange patch on one side and a black patch on the other, giving her a distinctive calico coat. She has large light blue sclera with narrow, pinpoint pupils, small pink inner ears, one of which is bitten off, a tiny black nose, and long black whiskers. They wear a bright yellow hazmat suit with black trim and a front zipper, blue safety gloves, a black belt, and black boots, giving them the appearance of professional hazardous materials workers.
Sam is apparently hardworking and experienced in handling hazardous materials, as he readily takes on such dangerous tasks. Despite being fired from Animal Corporation, he continues to work in a similar position at Animal Hospital, which speaks to his practicality and willingness to take on challenging work. However, they are quite clumsy, as over the course of three shifts (if hired), they could trip over their own boots, splash themselves, or accidentally drop a mop.

It can be purchased for 120 Animal Coins in the class shop in the server lobby. While using this class, you'll restore 1 Sanity point for each patient registered to you.

The Secretary is a blue, dog-like creature with drooping ears, wearing a business casual suit, a black tie, and brown shoes. They have large eyes with prominent black eyelids, oversized round black glasses, and a small, downturned mouth.

Secretary sheep are lavender-purple sheep of an unspecified breed with slightly drooping ears that are pink on the inside. They wear large black glasses and a white collared shirt with an orange tie. They have a shock of wool on their heads, reminiscent of an afro, and a small tuft of fur protrudes between their eyes. Their wool is slightly darker than their skin.

The Intern is one of the game's playable classes. It's the default class when you first log in.
When choosing this class, the player receives a 10-point bonus to Sanity at level 1, giving them 71% Sanity instead of 61%. At level 2, the Intern begins the game with 76% Sanity. Finally, this class's Sanity reaches 81% at the start of the round.
For a starting class, the Intern is quite good and ideal for newcomers. The extra health allows newcomers to spend a little more time learning the game. If other classes on your team have low Sanity, the Intern might be a good idea to suggest they grab some coffee early. The Intern is a rabbit-like creature with protruding ears. In the class shop, it's depicted as light orange, but changes color depending on the players during shifts. It wears a white hoodie with gray pockets, a zipper, and black drawstrings, along with simple white pants. Its face has small black dots for eyes and a wide smile.
When playing with other users, the Trainee can be green, blue, pink, or light orange.
Barney
Barney is a minor character in Animal Hospital. He first appears between shifts 2-7. Appearance
Barney appears as a white, rabbit-like creature with black, slit-like eyes (only visible when you let him into the office and an officer arrives) and a mustache. He wears a brown hat and plaid brown pants. His Anomaly form has a large mouth with sharp teeth (appears when an officer arrives) and large, black, empty eyes with a small white dot in the center. He holds a knife in his hands (when an officer arrives). Little is known about Barney's past. Barney is an Anomaly and a dangerous serial killer. Barney first approaches the player asking for coffee because he's had a long day. Considering he's a serial killer, it's safe to assume his long day consisted of murder. The next time he comes to the hospital, he asks for coffee again. If you close the shutters on him instead of giving him coffee, he will threaten you and say, "Watch out, I don't like being turned down." He'll say the same thing if you don't give him what he wants within 120 seconds. The aforementioned phrase hints at his being a serial killer.
In the following dialogue, he asks the player to keep an eye on his red briefcase. Barney will ask for more coffee the next time he comes by, and shortly afterward, he will ask the player to take a photo of him. It's unknown why Barney wants this photo, but it marks the first time the player sees Barney in his anomalous form. (Note: Officer Duckman will later show you this photo. How this photo came into Officer Duckman's possession is unknown.)
After the previous interaction, Barney will approach you and ask for coffee. After you give him the coffee, Barney will ask for a scalpel. This is the last shift during which you'll see Barney in his normal state, and during the next shift, he'll ask you to hide with him. You can refuse this request by closing the shutters, like all other interactions.

Barney asks for more coffee and shortly after asks the player to hide him. He is known to use a kitchen knife to kill his victims, as he did to incapacitate Officer Duckman. The police track Barney down, and he has little choice but to ask the player for help. The player can either let him hide in the office or close the door behind him until he leaves. Barney's refusal of help during previous visits will not affect this interaction. He will still ask the player to hide him and will still reward you with an extra coffee machine if you help him. If you hide Barney without revealing him, he will reward you with an extra coffee machine with a longer cooldown of 300 seconds. Personality
At the beginning of the game, Barney comes across as a very friendly and respectful character. He refers to the player as "friend" in many of his dialogue choices, and whenever the player helps him, he either thanks them or compliments them (e.g., "You're the best..."), mostly over coffee. In almost every message, Barney calls the player a friend, demonstrating a genuine connection with them. Despite this, he exploits the player to the fullest and is very demanding: he becomes upset if the player refuses to give him what he asks for. He is highly dependent on the player and feels like a parasite.
When Officer Duckman arrives at the hospital and his appearance changes, he tells the player that "friends don't rat each other out to the police..." and even adds, "If you talk, I'll kill you, friend..." So, while Barney can be kind, he gets angry when the player doesn't give him what he wants. He's a true sadist, considering he's a serial killer, and he won't hesitate to kill if he has to.
Ratthew
What does he look like?
Body and face: A gray-brown rat with typical rat features—an elongated snout, small ears, and a long tail. The eyes are the same slit-like eyes that characterize the game's style, with white irises.

There is a white ring around her eyes and on her eyelashes. She has a cream-colored muzzle, cream-colored whiskers, and a mouth shaped like a small upside-down V with the ends curled up. She has a thin brown tail that matches her fur. She wears a light blue cap, a black shirt, and a dark blue jacket. She has a white camera around her neck. She wears light gray pants and black shoes.
Liz used to work for the Animal Corporation news channel, but was fired for reporting on an anomalous infection, which the corporation dismissed as a "fake".[2] She asks the player to help investigate the case using a special UV camera to take several photographs of specific patients and include them in an article she plans to publish as proof of her case.[2]
Personality: As evidenced by her dialogue, Liz is always exaggerating and full of energy. She sometimes drawls out her words, saying things like "Oh-oh-so" and adding emojis like "😀" to some sentences.[3] She is quite friendly and trusts the player enough to allow her camera to be used for missions.
Liz is smart and determined, having discovered the truth behind the anomaly outbreak herself. Instead of remaining silent, as Dr. Harlow said she should have,[4] she attempted to expose Animal Corporation, despite the risks, for which she was fired. Even after her dismissal, she refuses to give up and continues her investigation with the player's help, hoping to prove her case.

In his right hand, he holds a black suitcase with the word "TOP SECRET" written in red letters, similar to Ron's suitcase from accounting. His face has slit-pupil eyes with shiny black irises, and his mouth is covered by a blue surgical mask. On top of his head are two large gray horns. He has dark orange patterns on his forehead, and his ears are slightly darker than his face. His appearance was updated in the "Harlow's Favorite Update".
