His starting item is a pistol, a black anthropomorphic cat with pointy ears. He wears a black waistcoat over a white collared shirt, a black tie, a white pocket square (napkin), and black trousers. The face features narrow eyes with bright yellow sclera. The mouth and nose are usually hidden. A funny detail: if you look at the character through a glass door, the mouth and nose become visible. Another feature: this is the only playable class in the game that does not have eye highlights with the standard eye model.

Void Body - the camera shows a completely black, empty body.

Twitching - the limbs twitch unnaturally, the neck arches back. Flesh Ceiling is a mass of flesh with eyes on the ceiling. If you look up, it will stare back and drain your sanity. The prompt "Don't look up" appears on the screen.

The Monster Under the Bed is a dark shadow with bright red eyes and enormous black hands. It hides under beds; a red line is visible next to the bed, making it dangerous to stand on.

The Stalker is a giant, rabbit-like shadow figure that peeks around corners. Direct eye contact will cause sanity to be lost.

Demon Cat - Visible only on cameras at the end of corridors; zooming in on it triggers a jump scare, causing a 20 sanity loss.

Ron works in the hospital's accounting department and knows far more than the average employee should: he's up-to-date on all the anomalies, monsters, and strange goings-on there. At the same time, he maintains a calm and pragmatic demeanor—as if he's already accustomed to danger. He resembles a blue creature, reminiscent of a rabbit or a dog (the exact animal isn't specified). He has dark brown fur, low-hanging dark blue ears, and huge round black glasses. He wears a white suit, black tie, black pants, and brown shoes; he always carries a black suitcase with the sign "TOP SECRET." He tells the player disturbing and important details about the hospital—for example, that the patients there aren't just sick, but infected.

The character is deliberately "a little off": the mask, strange eyes, and secret briefcase immediately make it clear that all is not well with the veterinary medicine at the clinic. Dr. Harlow appears at the beginning of shifts and gradually grants access to important mechanics: He seems to be trying to keep the clinic under control, but he clearly knows more than he's letting on. The "TOP SECRET" briefcase and evasive phrases hint that the clinic is a cover for some kind of experiments.

There are fan theories that Harlow himself may be an anomaly: in the lobby, he can be seen in a window in a "distorted" form, and in some promotional art, he is depicted with a knife and a frightening face.

He is not hostile to the player, but he is not a "kind mentor" either: rather, he is a person (or not a person) who is forced to work within the system and tries to minimize the damage. Dr. Harlow is actually a villain who has been bothering us, but no one knows about it.

Ratthew in Animal Hospital is a mysterious rat character who plays a vital role in the game's lore. He's a cartoonish rat in a slightly tattered white lab coat.

He has large dark eyes and a long tail that is almost always twitching.

Often holds in his paws a tattered notebook or folder with documents, where scraps of notes are visible.

In some scenes, stains and red marker marks are visible on his lab coat—a hint that he was part of the experiments. Ratthew is one of the few who knows what's really going on in the clinic. According to fan theories (and hints in the documents in the safe), he's either a former lab technician or a test subject who managed to retain his sanity. His goal isn't to defeat the monsters, but to preserve and transmit information. Therefore, he's cautious, timid, and always in a hurry.

Officer Duckman is a police duck in Animal Hospital and plays a major role in Barney's storyline. What does he look like? Black duck (stylized, cartoonish).

He wears a bright blue shirt, black trousers and a black tie.

On his head is a blue police cap with a visor, and in the middle is a silver badge.

For example, when it comes to making an arrest, he doesn't draw his weapon, but simply yells for Barney to be arrested. This adds to the absurdity and horror-comedy: something terrifying seems to be happening, but the police act as if it's just another shift.

Barney in Animal Hospital isn't your average assistant, but rather an iconic boss anomaly (a serial killer, according to the lore) in the form of a white rabbit. A white rabbit with a mustache.
