When Officer Duckman arrives at the hospital and his appearance changes, if the player speaks to him, he says that "friends don't betray each other..." and even says, "If you talk, I'll kill you, buddy..." So, while Barney can be friendly, he completely loses control when the player doesn't give him what he wants. He is quite sadistic, considering he is a serial killer, and has no qualms about killing if necessary.

They take the item you are holding and reduce your sanity by 10 points if you are unarmed. They can be repelled with a fire extinguisher or taser. Mass of Eyes: A mass of flesh and eyes stuck to the ceiling of the rooms that damages your sanity if you look at it. It can be treated with eye drops. Head Banger: A patient with hollow eyes who bangs his head against the reception glass. You shouldn't use the taser on him; to make him leave peacefully, you must give him a coffee. Camera Figure: Appears when you zoom in on a broken security camera. If you don't close the monitor quickly, it will give you a lethal jumpscare. Tendril: A huge purple mass with tentacles that invades Room 8 during surgeries. You have to finish the operation before time runs out.

In rooms 1-5 there is a stretcher, a DNA machine which analyzes the patients' blood, a computer where the results are displayed, and a large screen where the treatment required by the patient is shown.

Note: Surgeries have a time limit, and you must give the patient the treatment requested on the screen on the wall. If you make a mistake, they will die.

His face features slanted eyes with white irises and prominent black eyelids, and a small, droopy mouth.
