Sage resembles a young human woman. She has a slender body and limbs with white skin, and a large, round head with a high forehead. Notably, she has glowing red eyes and a red energy structure on the left side of her face. She also has short white hair with a swirl on her forehead and a single, prominent point of red energy within that swirl. However, the rest of her body, especially her arms and legs, is completely black and appears to have red computer code outlines around them. She wears a black cape-like dress and black Mary Jane shoes.

Sage is demonstrably expressionless and indifferent. She is only interested in following her objectives and orders, disregarding the objections of those who oppose her, which leads her to have a somewhat tsundere attitude—serious and sometimes even angry. She struggles to understand human behavior or behavior outside of artificial intelligence, referring to Sonic as "stubborn" for refusing to leave behind his friends who need him. Similarly, she shows curiosity regarding the bonds people can form with their close friends. Ironically, she does possess emotions of her own, with her primary loyalty being to her loved ones, her most prominent bond being with her creator, Dr. Eggman, whom she considers her father. Likewise, she sympathized with the strong and unbreakable friendship between Sonic and Tails, formed by true solidarity and not genetics. According to Eggman in Memos (#18), Sage told a joke about Sonic's speed and how he can ruin everything "instantly," implying that even as an artificial intelligence, she has her own sense of humor. In The Final Horizon, it is shown that unlike Sonic, he is not very interested in saving the planet out of compassion but because Dr. Eggman wants to conquer it.

As for her undergarments, she wears a two-piece lingerie set that follows a similar tech theme. The set consists of a top and panties with a black diamond pattern accented by bright neon red lines. Her thighs are covered with long stockings that continue the tech design of black diamonds and luminescent red lines.

Sage was an AI created by Dr. Eggman, frustrated by the failures of his other creations, such as Orbot and Cubot. He managed to create an artificial intelligence far more advanced than any software he had previously created, naming her "Sage." The first task he assigned her was to hack into the technology of the ancient civilization inhabiting the islands to uncover their secrets. Despite Sage's warnings about the dangers, he pressed on, confident of overcoming them. When Eggman installed Sage on one of the altars, she encountered a problem that dragged both her and Eggman into cyberspace.

After being infused with data from Cyberspace, she learns all about the civilization that once inhabited the Starfall Islands, the Ancients, and discovers that a powerful being called The End had been sealed away by them in Cyberspace by powerful robots called Titans after he tried to wipe out his entire species and prevent him from continuing to destroy other worlds.

While this was happening, Sonic, Tails, and Amy were also pulled into Cyberspace by The End through a wormhole. During this time, Knuckles the Echidna found an ancient portal in the ruins of the Celestial Sanctuary and traveled to the Starfall Islands. Knuckles fought and defeated a group of Guardians until Sage intervened, sending an Asura after him. After capturing him, Sage imprisoned Knuckles. Sage also imprisoned Tails and Amy.

Sage later states on Ouranos Island that he did this to ensure Dr. Eggman's safety while he was trapped in Cyberspace by getting rid of external variables, as not all of them were meant to come to the Islands.
