The first part of his name, "oto" (音) means sound in Japanese, "hiko" (引込) means "retraction", and "Meichi" (灭痴) in simplified Chinese means "idiot". He has a different running style than other students, as he holds his arms slightly raised. Otohiko has an extremely traditionally masculine father, who is disappointed in his son for considering him "effeminate" and/or "effeminate." The nurse knows him well and treats him kindly, having come to expect his frequent visits. The infirmary has become a second home for him, a peaceful refuge where he can momentarily escape the pressures of school life. Otohiko's fragile health has another consequence: he needs more sleep than the average person, a characteristic that has caused him many problems. Most mornings are a challenge for him; he finds it almost impossible to wake up on time. As a result, he often stumbles into the school building moments before class starts, looking disheveled and half-asleep. His teachers and classmates already expect his tardiness, and many are surprised when he manages to arrive punctually. For Otohiko, simply getting to class on time is an achievement in itself. Otohiko's delicate voice and gentle tone have earned him ridicule from some of the more intolerant students at Akademi Academy. Otohiko has long, salmon-colored hair. A large strand runs down the middle of his forehead, covering the space between his eyes. Another strand of hair runs to the left on top of his head. His hair hangs down to one side of his head and stops slightly above his ears. His eyes are a bright peach color.

A frail and sickly boy. Due to his poor health, he can often be found at the school nurse's office. He tends to oversleep and arrives to school later than any other student. He is sometimes ridiculed for his unusually feminine mannerisms. He is known for being extremely clumsy.

Otohiko is a coward. If he sees a dead body, he will run away from school and call the police. If he witnesses a murder, he will begin begging the murderer to spare his life in exchange for his silence. Because he is a coward, he will hide his face if someone points their phone at him. He cannot engage in physical fights with murderers. If he finds a bloodstain, a weapon, a dismembered body part, or a bloody uniform on school grounds, he will run to tell his teacher. If he comes across a non-suspicious weapon, he will grab it and return it to where it belongs. It is possible to frame him with this information.
