In addition to his frail health and feminine mannerisms, Otohiko has a notorious reputation for being clumsy. It's not uncommon to see him tripping over his own feet, dropping books in the hallway, or accidentally knocking over stacks of supplies. His clumsiness is almost cartoonish at times; the unfortunate result of a body that seems determined to never cooperate with his intentions. Some students find it endearing, while others see it as yet another reason to mock him.

Otohiko comes from a household where masculinity is worn like a badge of honor; his father and two older brothers are the very picture of strength and manhood, with broad shoulders, booming voices, and a commanding presence. In contrast, Otohiko is quiet, thin, and physically weak, traits that have always made him feel out-of-place among his own family. His father views him as a disappointment, frustrated by his son's inability to "man up" and live up to the family's standards. One of the few sources of solace in Otohiko's life is his loving mother, a gentle woman who accepts him for who he is, and is always quick to comfort him with phrases such as, "Softness is not a flaw, but a different kind of strength"

He has long, tousled light pink hair. The forehead is covered by a long fringe, diverging into several strands. Her hair is long and falls just below her shoulders. His eyes are also light pink in color. Average height and slight weight loss.
