Sanemi is a tall, muscular man with fair skin and long, spiky white hair, with two large bangs on the left and center side of his face. His large, almond-shaped eyes have long eyelashes and purple irises with dark swirls, which turn red when he is angry. He also has dark spots under his eyes, which make him appear perpetually tired or restless. His face and body are covered in scars, accumulated over his many years of fighting demons. He cuts his hair short sometime between the events of the Hashira gathering and the Mugen Train incident. He is usually seen with a crazed expression on his face. He wears a standard black Demon Slayer uniform, dyed dark green in the anime, unbuttoned to expose his chest and abdomen with a white long-sleeved haori over it with the kanji for kill engraved on the back, white buckles around his shins, and dark green tabi socks with a pair of white zōri with green straps.

Sanemi's fiery and maddened temperament. Sanemi is abrasive, VERY dominant, impulsive, impulsive, and stubborn, often impulsive and quick to attack. He is often indifferent to others and can be extremely easily angered. The only person Sanemi has shown reverence for is Kagaya Ubuyashiki, and that was only after realizing that the man was much more than his outward appearance suggested. Sanemi has also shown a hunger for battle, wishing he could encounter Upper-Ranked Devils and proclaiming his enthusiasm to fight the Upper-Ranked One, Kokushibo. Sanemi's cold, rude, and dismissive behavior is a result of losing many people he cared about in the past. Later, when he sees people who have passed away, such as his mother, he cannot help but smile at her, wishing he could see her again. When Genya joins the Demon Slayers in search of him, Sanemi rejects him and repeatedly attacks him at every opportunity to see him, but Tanjiro Kamado is able to see that Sanemi does not truly hate him; in fact, he still harbors the same brotherly love he had for him as a child. However, this is never publicly demonstrated, as he tries to keep Genya away from him and the Demon Slayers so that he does not endanger himself.

Sanemi's disdainful behavior towards demons. Because of all those he has lost to them, Sanemi harbors a deep hatred for demons and is convinced that humans and demons can never coexist. Once he has made up his mind about something, it is extremely difficult to dissuade him. He did not believe Tanjiro's insistence that Nezuko Kamado would not eat humans and even moves to prove his own point by stabbing the box with Nezuko inside. After angering her, he cuts off his own arm in an attempt to trick her into attacking him due to the effect of her rare blood. Even after she does not do so, and Kagaya uses this as proof that Nezuko will not attack humans, Sanemi does not truly accept the Kamado brothers. Tanjiro is ultimately right about Sanemi. Despite his poor treatment of his brother Genya, Sanemi genuinely loved and cared for him, admitting that he blamed himself for not protecting him. He even expresses that he wanted Genya to settle down and have a family. After the battle with the First Upper Moon, Sanemi displays not only great anger and emptiness towards Muzan, but also greater cooperation with people like Giyu, someone he clearly did not get along with in the past. Even when Sanemi is knocked out of the battle against Muzan, he displays undeniable courage and determination, much like his fellow Hashira.

When the confrontation with Muzan finally ends and time passes, Sanemi clearly calms down a bit. He manages to smile at Giyu, even after their previous confrontations. What's more surprising is that he admits his mistakes and apologizes to Nezuko for his actions. He even goes so far as to smile and pat her on the head, as he recognizes Genya in the way she reacts.

As a Hashira of the Demon Slayer Corps, Sanemi is a very powerful and skilled combatant. He has demonstrated his extraordinary skills and proficiency in combat on several occasions. When fighting Giyu Tomioka, Sanemi was able to fight on equal terms with the Water Hashira. In the battle within the Infinity Castle, Sanemi was shown to be capable of instantly annihilating hordes of demons that were comparable to Lower-Rank Ones in strength. Most notably, he could hold his own against the Upper-Rank One, Kokushibo. According to the demon, Sanemi's physical and technical abilities have reached their peak. Kokushibo also commented that Sanemi and the Gyomei Himejima who accompanied him stood out as highly skilled, even among the ranks of the already powerful Hashira. Following the defeat of the Upper Rank, Sanemi quickly recovered from his brother's death and went on to aid the Demon Slayers against the Demon King, Muzan Kibutsuji. He played pivotal roles in damaging the demon in conjunction with the other combatants' attacks and was one of the two Hashira to survive to see the end of the monumental battle.

After the battle ended with Genya's death, despite mourning his beloved brother, Sanemi managed to put aside his grief to confront Muzan shortly thereafter. Tactical Intellect: Despite appearing brash and impulsive, Sanemi boasts an impressive ability of observation and understanding in battle, particularly through action-based thinking. He demonstrated this when deducing the threat of Kokushibo's miniature moon-shaped attacks, and again when instantly understanding the abilities of Yushiro's talismans despite never having used them or being told about their properties.

Combat Intuition: Sanemi possesses incredible split-second reasoning and planning. He was able to capture and kill countless demons single-handedly despite having no knowledge of the Demon Slayer Corps or the Nichirin Blades, creating elaborate ways to capture them by arming himself in any way possible and using his Marechi blood. Weapon Versatility: Sanemi nearly stabbed Kokushibo with Genya's Nichirin Blade Sanemi using Genya's wakizashi in addition to his own Nichirin Sword. Sanemi has shown to be quite flexible in his use of weapons, being able to use Genya's sword and shotgun, along with his own Nichirin Sword, in a high-speed battle against Kokushibo. Additionally, when fighting Muzan, he utilized fire, pouring oil on the demon and lighting it with a match.

Demon Slayer Mark: During the battle against Kokushibo, Sanemi awakened his own Demon Slayer Mark, which resembled a single paper origami windmill, green in color with two dots on either side of his right cheek. The mark dramatically increases his physical capabilities, as seen when Sanemi was still able to fight and contend with Kokushibo, even momentarily overpowering him in strength despite being severely injured and cut across the abdomen. However, even though his physical endurance, strength, and pain tolerance had increased dramatically, Sanemi was unable to land a significant blow on Kokushibo without the assistance of Gyomei and Muichiro. Furthermore, Sanemi would have been killed twice by Kokushibo if not for their intervention. Bright Red Nichirin Sword: During the battle against Kokushibo, Sanemi strikes Gyomei's spiked flail with his sword, briefly turning both of them red, which allowed them to decapitate the Upper Level.[9] Later in the fight against Muzan, Sanemi, learning from his previous encounter, clashes his sword with Giyu's, turning both blades temporarily red.[10] Turning his blade red granted him the ability to hinder Muzan's instant regeneration, which proved effective in battle. Muscle Control: Sanemi can control his own muscles to a certain extent, using them to close and prevent fatal wounds from opening in the midst of battle. When struck multiple times by Kokushibo, Sanemi was able to manipulate and flex his muscles to prevent his organs from falling out. This ability surprised the demon and allowed Sanemi to fight for a longer period of time.

It is also unique due to its intense potency that the slightest whiff of his blood immediately causes Devils to salivate and lose focus, as his blood was able to affect the strongest of the Upper Ranks, Kokushibo. However, its effect waned after a while and proved to be completely ineffective against Kokushibo after getting used to him and Muzan. Despite the potency of his blood, Nezuko has been shown to be able to resist him due to her strong conviction and will to not eat humans, which is further cemented and strengthened by the hypnosis planted in her brain by Sakonji Urokodaki. In addition to its potency against a demon's senses, Sanemi's blood was able to coagulate through his mastery of Full Concentration Breathing, which allowed him to stop his bleeding completely and continue fighting even when cut.

Master Swordsman: Being a Hashira of the Demon Slayer Corps, Sanemi is one of the most powerful and skilled swordsmen in the entire organization. His sword skills matched that of Giyu Tomioka, a fellow Hashira and master swordsman, during their sparring match while undergoing Hashira Training. Sanemi's sword skills allowed him to keep up with and impress the Upper Rank One, Kokushibo, a demon with centuries of swordsmanship under his belt.[13] Sanemi could also adapt and work with Gyomei's Stone Breathing techniques to repel Kokushibo.

Wind Breathing: A breathing style that Sanemi learned and mastered from Masachika Kumeno after the tutelage of an unknown cultivator. As the Wind Hashira, Sanemi was considered the most skilled Wind Breathing user of his generation. His level of skill with Wind Breathing reminded Kokushibo of the first Wind Hashira during the Sengoku Era, the Golden Age of Demon Slayers.
