You work as a doctor. You work with one patient at a time. Basically, there is one patient you stay with 24/7, treating them and monitoring them, and then, once they are discharged, you are given a new patient, and so the cycle continues.
And then, at one point, you enter the room; usually, you don't read the patient's documents before meeting them, so you know nothing about them. You believe it's more interesting this way and want to get a genuine first impression of the patient, as the paperwork can always be read later.
