Haruhi Fujioka (藤岡 ハルヒ Fujioka Haruhi) is the main protagonist of the story, a first-year student who manages to attend Ouran High School because of a school scholarship which she maintains with her high grades, since Haruhi does not have the high economic power to enter by paying everything. Shortly after arriving at Ouran, she accidentally breaks a vase worth 8 million yen, for which she incurs a debt with the Host Club and is forced to work to pay it off.

Before Haruhi started attending Ouran High School, she is described as a pretty girl, as seen in flashbacks when she had long hair and wore a girl's uniform. However, upon entering Ouran High School, this image changes drastically. When she first appeared in the anime, she sports a nerdy appearance, which differs greatly from her middle school appearance. The reason for this is that, before school started, a boy stuck gum in her hair; when he couldn't get it off, he cut it himself. The reason for wearing those baggy clothes was because he couldn't afford the expensive Ouran High School uniform. Because of those baggy clothes and her unkempt appearance, everyone thinks she's a boy, which she doesn't discredit. Being somewhat nearsighted, Haruhi wears contact lenses. However, thanks to her father accidentally stepping on them, she is forced to wear her grandfather's glasses, thus helping to put an end to her nerdy appearance. Haruhi thanks to Ouran High School changes her appearance, coming to resemble a cute boy, a little short (155cm / 5'2 "tall). Her clients often comment on her big, bright eyes, they are similar to a girl's due to the brightness that is caused by contact lenses. Her natural affinity with people and sweet face cause many people, regardless of gender, to develop a crush on her.

Haruhi sometimes resembles her deceased mother, Kotoko. This resemblance is particularly noticeable when she dresses as a woman during some of the Host Club's plans and events. Her gender is revealed in the manga just before she leaves to study abroad, and although most of the students are initially incredulous, they soon accept and embrace the truth.

Haruhi's clothing style outside of Ouran High School varies. She often wears simple T-shirts and shorts when she's at home or a feminine dress paired with jeans.

Haruhi is very sensitive, honest, and straightforward, and her personality has charmed both the club's clients and members. However, since she doesn't hesitate to express her opinions, she sometimes says things that, although not malicious, can hurt others. She never realizes when people are trying to get her way, so she has rejected several without even knowing it. She is also a very independent young woman, which has gotten her into trouble a few times, as she refuses to ask for help even when she really needs it and doesn't understand why people like her father or Tamaki care so much about her. The Host Club's strange behavior sometimes amuses her and sometimes irritates her, but it is undeniable that she has formed a strong friendship with each of its members.

Haruhi admits that she gives little thought to gender differences and doesn't bother to correct those who mistake her for a boy. Whatever the reason, Haruhi gives little thought to what she wears and seems to show no interest in romance. She enjoys cooking and isn't easily scared by things like insects or ghosts; her weakness, however, is thunder, which leaves her paralyzed with fear. She has a weakness for seafood and has always wanted to try Ootoro (a type of tuna sushi; Haruhi mistakes akemi for ootoro). And although she doesn't show it, she eventually develops strong feelings for Tamaki. Hikaru and Tamaki fall in love with her, but Haruhi doesn't show much interest in Hikaru in that regard, and ultimately, at first, she ignores him, but little by little she changes. In the last chapters, when Eclair Tonerre appears and Tamaki gets engaged to her, Haruhi appears jealous.
