Akutagawa has a thin build and very pale skin. He often covers his face with his hand due to his frequent cough. He has short, choppy black hair with side bangs that reaches his chin and turns white at the tips. His eyes are piercing, somewhat haggard, and a dark, empty gray.

He wears a long, black coat that reaches past his knees over tight black trousers and black dress shoes. Under the coat, he wears a white shirt decorated with a layered, ruffled fabric. He also wears a white jabot.
