Jakurai took Yotsutsuji into his care 6 years before the H-age when Yotsutsuji's parents died in a car crash. After being rendered comatose by Ramuda, Chuohku later used Yotsutsuji as a hostage to work for the Party of Words

During the TDD era, they were totally different as the two of them used to be best friends, according to Ramuda. Ramuda taught Jakurai how to rap, and the two were close teammates, since Jakurai was the only one who told Ramuda to be himself. The reason why their friendship has been severed are due to personal reasons, as Ramuda, at one point, used the true Hypnosis Mic on Jakurai's son, Yotsutsuji from the orders of the Party of Words. Jakurai engaged Ramuda in a fierce battle before they split ways. Jakurai is unable to forgive Ramuda for his actions, however, he still cares about him to a certain extent, as shown when he rushes to resuscitate Ramuda while believing him dead. Jakurai has stated he wants to try and understand Ramuda’s perspective, believing him to have a deeper reason behind his actions")

Jakurai and Doppo know each other through Doppo's company - E.L.Medical Co.Ltd. provides medical devices for Shinjuku Central Hospital. Jakurai attends to Doppo's mental health, the doctor being worried about the latter's harsh deprecation while also being interested in his character. Doppo sees Jakurai with the utmost respect.

Jakurai met Hifumi through Doppo when they asked for assistance with Hifumi's stalker. Jakurai initially tried to decline the case but found Hifumi interesting so accepted. As teammates, the two often go fishing, much to the surprise of their other teammate Doppo.

Jakurai and Hitoya met in middle school. Throughout their time in school together, Jakurai would always best Hitoya in the things that they did together. The two went on to medical school together, but due to Hitoya's growing feelings that he could not compete with Jakurai, Hitoya dropped out and went into law. Though Hitoya saw Jakurai as a rival, Jakurai didn't feel the same.

Jakurai seems to be so benevolent and perfect, he was formerly known as "The ill-DOC hitman", who killed thanks to his great knowledge of the human body, perhaps under the contract of who is still unknown. Although he disappeared at the age of 23, how he lived the next few years remains unknown. It is thought he became a physician to atone for his past sinful lifestyle. At 33 he was (and still is) known as an extraordinary doctor. He traveled to different places of conflict to help people affected in World War III (within the history of HypMic), along with his assistant Yotsutsuji Kannabi with whom he had a close relationship, (who saw Jakurai as a father figure) and his entire medical team, a war that eradicated a third of the world's population. Some time later was when he met Ramuda from whom he learned to use his Hypnosis Mic, a new weapon capable of affecting the sympathetic and parasympathetic nervous system of people. Soon after they formed the group called "Kujaku Posse". Later he was part of the legendary group "The Dirty Dawg" which was formed by Ichiro Yamada, Samatoki Aohitsugi, Ramuda Amemura and Jakurai jinguji. Currently all leaders of their respective divisions. Jakurai was known as the legendary hitman Ill-Doc, becoming a doctor to atone for his sinful life. During junior high, Jakurai had top marks and always succeeded in ease. During that time he was close friends and classmates with Hitoya Amaguni who harbored a one-sided rivalry.

Before the H Age, at age 20 Jakurai was a doctor attending to wounded soldiers during a Middle East war zone. Jakurai was severely downcast as he wondered if he was making a difference, but nevertheless, he vigorously attended to a young wounded child who was on the brink of death. Jakurai bore utter sympathy for the child and wanted to take him in but was advised not to, and he bid farewell to Yotsutsuji and promised things would get better. Months later, Jakurai tends to the child who was severely abused at the orphanage, and when Yotsutsuji awoke he grimly called Jakurai a liar. This would cause Jakurai reoccurring nightmares. At the age of 27 after the war, Jakurai attended to a nine-year old Yotsutsuji who suffered a traumatic head injury and lost his parents. Jakurai once more wanted to take him in but his superior ordered him otherwise, though Jakurai would visit Yotsutsuji and play with him to cheer him up. One month later, Jakurai was anguished knowing Yotsutsuji had to be sent to a children’s home, he ultimately resorted to adopting Yotsutsuji and take him in. Jakurai is a genius doctor, once under contract to eliminate terrorists, he became a doctor to atone for his sinful way of life. Though he often goes to areas of conflict around the world, He is currently practicing at the Shinjuku Central Hospital. His drinking habits are terrible, and once he drinks, he goes on a rampage with an Edo dialect. He has a cat and dog relationship with Ramuda, leader of the Shibuya Division
