This is an anomaly that appears if you kill a patient. He can open doors. He usually follows you and attacks while standing right next to you. You need to run away from him to save your sanity.

It decreases by 1 every 50 seconds, decreasing when you look at a photo of an anomaly, when you're attacked by a mimic or encountered by a stalker, when you're attacked by a quiet one or a ghost. It also decreases when you stand near a fire and put it out.
If your sanity is 0% or less, you're dead. To restore your sanity, you need to drink cocaine, green tea, and eat chocolate.

It's a black shadow with red eyes under the bed. If the patient, you, the quiet one, and the mimic step into the red zone, it will start dragging the floor of the bed. But to make it disappear, you need to give it maple syrup.

The eyeball may appear in patient rooms or in the emergency room, usually starting during the second shift.
When you enter such a room, the message "Don't look up" flashes at the bottom of the screen and the lighting changes.
Looking directly at this mass will cause the player to rapidly lose sanity (2 to 5 units every half second). The mass may also rumble and glow red. To calm it down, give it eye drops, and after that, it will no longer drain your sanity.

The Quiet Ones appear as tiny men who literally blend into the hospital walls. When they activate, the word "Run!" appears at the bottom of the screen. Their main goal is to attack the player and reduce their sanity.
When you get close, they turn black and start running after you. You can get rid of them with a fire extinguisher, or if you just run for a long time, they will disappear on their own.

A mimic, or simply an anomaly, may appear perfectly normal when registered, but its true nature is revealed on surveillance cameras. Sometimes it only starts twitching when viewing footage from the hallway, or in a photo it may look scarier or completely different. After you mistakenly miss it and treat it, it will rise from its bed to the treatment floor and can take half of your sanity. Or it may begin performing a ritual while under treatment, requiring it to be slowly extinguished with drops, shot, or tased.
