There are two factors to check:

1. Photo. If the photo differs even slightly from the patient, it is an anomaly.
2. Cameras. You need to look at the CCTV cameras. If the patient is normal, everything is fine. If, for example, they are looking directly into the camera (THIS IS ONLY VISIBLE ON THE CAMERAS) or have a very strange shape (ONLY ON THE CAMERAS), then this is an anomaly.

Normal ones - they are not aggressive, they just need to be treated and that's all.
Skinwalkers can attack, with a 50/50 chance. They can be scared off with a pistol or a stun gun, after which they will simply flee the hospital.
Ritualists – they often have empty black eyes. After their treatment, they can perform a death ritual in their chamber. It can be stopped by extinguishing the candles.
The "Three Eyes" anomaly—they're also skinwalkers. But their attack chance is increased. They have three red, slightly protruding eyes.
