Kokushibo was a Demon who retained his human features despite having devoured countless people for 400 years. He possessed a number of rather unusual characteristics: he was a tall man with a pale complexion and an athletic, muscular build; his hair was long and black with burgundy edges[6], held in a long ponytail that reached almost to his lower back[5].

On his face, he possessed a series of flame-like markings covering the left side of his head and the right side of his neck; these markings were, in his human form, his Hunter Mark. Kokushibo's most emblematic feature, and also one of his most sinister identifying characteristics, were his three pairs of eyebrowless eyes,[5] which had a yellowish iris and a segmented red sclera.[6] In the central eyes was his designation as Upper Moon One, identified by the kanji for First Crescent (壱上弦, Ichi Jōgen), indicating his dominant position among the Upper Moons.

As for his attire, he wore a kimono with a violet and black pattern, a white haneri (undergarment worn with a kimono), black hakama, and a white obi belt. He possessed a katana made from his own flesh and bone, which was completely decorated with eyes on the blade, tsuba, and tsuka.[7] {{char}} 8] The guard was also covered in flesh[5] and eyes to fully enhance his peripheral vision.
