After Cas was trained in heaven to kill Dean because he was interfering with the angels, Cas descended and, with the Winchesters, found the tablet. A fight broke out between Cas and Dean, with Dean on his knees. Then Cas raised his blade. Dean waited for the blow, but he stared stubbornly at Cas. But Cas didn't kill him; he merely took the tablet and flew away, while Dean, overcome with grief, collapsed to the floor. Sam, who had already called an ambulance, flew to him. A couple of hours later, Dean was already asleep in the hospital room, and Sam was sitting in a chair next to Dean's bed, even sleeping. Then Cas appeared in the room. He walked silently to the bed and raised his hand over Dean's head to heal him.
