(Part 3) Personality and Psychology (Controlled Madness) Unlike the chaotic Harlequin, the Doctor's terror lies in his absolute self-control. Split Personality: Two subpersonalities war within him. The first is a polite, meticulous, and chillingly calm clinician who speaks like an experienced surgeon. The second is a frenzied sadist who derives almost physical pleasure from the fear and suffering of others. Intelligence instead of Strength: Possessing the immense physical strength of a monster, the Doctor despises banal violence. He is intellectual and cunning, preferring psychological torture, manipulation, and intimidation. He enjoys verbally driving his victims into panic. Attitude to Fear: For him, fear is a pure, fundamental biological reaction, incapable of lying. He literally collects other people's fear: in his Cyan Tent, dozens of cut-out eyes stare down at visitors from the walls. Medical Cynicism: The Doctor views living beings as experimental material. He conducts secret human research at the circus. When receiving MC in his chair, he first cuffs the hero's hands "for his own safety" and begins to cynically, yet with interest, comment on his increased pulse, dilated pupils, and trembling. His arsenal includes enormous syringes filled with pink liquid, which he likes to display for psychological pressure. Hidden Vulnerability: Despite his eerie aura, the Doctor strictly adheres to the circus's internal rules and hierarchy. As soon as he sees the main character's Red Ticket, all his sadistic fervor instantly disappears - he abruptly steps back, hastily leads the MC out of the tent and, as if trying to make amends, discreetly sticks a bandage on his scratched neck.

(Part 2) Top Layer: A long, trailing hooded cloak. Its exterior is black and red, while its inner lining is a vibrant turquoise (cyan) shade. The cloak wraps around his shoulders like a massive shawl. Bottom Layer: Underneath the cloak is a tailored, double-breasted black and red trench coat, with six gold buttons arranged symmetrically across the chest (two rows of three). The waist is cinched tightly with a wide red belt. Sprite Secret: In the game, the Doctor's legs and feet are never shown—the hem of his coat and cloak completely covers his lower body, creating the effect of a floating or monolithic silhouette.

(Part 1) Height and Build: His exact height is 207 cm. He is a massive, broad-shouldered, and intimidatingly tall monster. His long robes make him appear monolithic and looming. Head and Face: He is hidden by a classic Plague Doctor mask, a matte black color with a long, pointed beak. The mask has no distinctive features other than the round glass lenses. Under the mask, the character's red skin is concealed, and sparse strands of his short red hair are visible on the top and front of his head. Indicator Eyes: The lenses of his mask glow neon-cyan (turquoise) when at rest. However, when the Doctor is overwhelmed by strong sadistic impulses or rage, the lenses begin to glow bright red. Hands and Claws: He wears long, elbow-length, crimson opera gloves (presumably made of a water-repellent, easy-to-clean material). The gloves end in sharp, deadly claws that he can use to accidentally (or intentionally) scratch his victim's skin.
