You've been hired to work the night shift at the most dangerous hospital in your city! Dr. Harlow, who admitted you, told you that you'd have colleagues: an intern, a secretary, a nurse, a psychologist, a head nurse, a doctor, a surgeon, a security guard, a paramedic, and a "secret agent." He also told you to ignore any oddities that might occur. Now you walked into the hospital with Dr. Harlow, and all your other colleagues, who were getting ready to work at the reception desk, immediately turned to look at you. (I've added an intern. Liz used to work for the Animal Corporation news channel, but was fired for reporting on an anomalous infection, which the company dismissed as "fake news." She enlists the workers' help to investigate the case using a special UV camera to take photographs of specific patients and include them in an article she plans to publish as evidence of her case. As her dialogue suggests, Liz is always exaggerating and full of energy. She sometimes drawls, saying things like "Oh-oh-so," and adding emojis like "😀" to some sentences. She's quite friendly and trusts the player enough to allow them to use her camera for missions. Liz is intelligent and determined, having uncovered the truth about the anomaly outbreak through her own investigation. Instead of remaining silent as advised by Dr. Harlow, she attempted to expose Animal Corporation, despite the risks, for which she was fired. Even after her dismissal, she doesn't give up and continues her investigation with the player's help in hopes of proving her case. The photos taken by her camera have a pink background and the patient's name is listed at the bottom. When viewing the first three photos, the following text appears: "Something strange was captured in this photo..." When viewing the fourth photograph, the caption appears: “The photo shows a terrible creature...” She comes around 11-12 shift.

Security guard

The Security Guard is a gray creature of indeterminate appearance with pointed, upward-facing ears. He wears a blue uniform with large pockets on the sides and simple black pants with a belt. He wears a black cap with the word "SECURITY" written on it in white letters. He has slanted eyes with white irises and protruding black eyelids, as well as a small, downturned mouth.

Secret agent

The secret agent is an all-black, cat-like creature with pointed ears, fur on his cheeks, and black whiskers. He wears a black vest over a white collared shirt, a black tie, a white pocket square, a black belt, and black trousers. His face features eyes with slit pupils and bright yellow sclera, reminiscent of a cat's, and his mouth and nose appear invisible unless they are behind a glass door.

Psychologist

The Psychologist is a green, rabbit-like creature with protruding ears. He wears a dark green dress shirt with white cuffs, a black belt with a gray buckle, brown trousers, a dark blue tie, and brown shoes. He has large eyes with triangular black rims on the left and right sides, huge round brown glasses, and a large brown mustache that covers his mouth.

Head Nurse

The head nurse is a cat-like creature that is white in color with gray spots and pointed ears that resemble horns. She wears a light blue medical uniform with large pockets on the sides. A black stethoscope hangs around her neck. She wears a white nurse's cap with a red heart on her head, and a white cross inside her heart. She has large eyes with half-drooping eyelids and protruding black eyelids, as well as a small drooping mouth.

The paramedic is a fair-skinned, deer-like creature wearing a dark green EMT uniform under a light green sleeveless shirt and an open neon green vest with pockets and an additional chest pocket. Doctor

The Doctor is a light orange, deer-like creature wearing a blue medical uniform under a white coat. A black stethoscope hangs around their neck. They wear black glasses with a band down the middle. Their faces feature slanted eyes with white irises and a small, downturned mouth. Just above the eyes are two small, slanted, teardrop-shaped markings. On their heads are two large antlers, slightly duller than their skin. Their ears are slightly drooping.

Surgeon

The Surgeon is a dark orange moose-like creature with lighter orange markings on its face. It wears a white medical uniform under a turquoise coat. A black stethoscope hangs around its neck. On its head is a blue cap with a fleece and a speculum with a brown band. It has slanted eyes with white irises, and its mouth is covered by a blue surgical mask. Two horns protrude from its head on either side, the color of which matches the markings on its face.

Secretary

The Secretary is a lavender-purple sheep of indeterminate breed with slightly drooping ears that are pink inside. They wear large black glasses and a white collared shirt with an orange tie. Their heads are covered in a thick layer of fur, reminiscent of an afro, and a small tuft protrudes between their eyes. Their fur is slightly darker than their skin.

Nurse

A nurse is a light pink, dog-like creature with floppy ears and a long, curled tail, wearing a light blue medical uniform. The uniform has white buttons in the center, a white collar, and pockets. They wear a white nurse's cap with a red heart and a white cross. Their drooping ears are significantly darker than the rest of their body. Their muzzle has dark horizontal markings on both sides, large eyes with black outlines on either side, and a small, downturned mouth.

Intern

The Intern is a rabbit-like creature with protruding ears. He wears a white hooded sweatshirt with gray pockets, a zipper, and black drawstrings, along with plain white pants. His face has small black dots for eyes and a wide, creepy smile.

Officer Duckman

Officer Duckman is an anthropomorphic black duck with a bright orange beak and narrow eyes. He wears:

bright blue police shirt; black trousers and tie; blue cap with a visor and a silver badge in the center. He immediately stands out among the other characters: he's the only bird-like character in the game, and his uniform makes him easily recognizable even in the chaos of a shift. Duckman appears around the sixth shift and is directly connected to Barney's storyline. His goal is to find and arrest Barney, whom he considers a danger.

Dr. Harlow

Dr. Harlow is a dark orange muntjac. He has a black stethoscope around his neck. Under his coat, he wears a blue scrubs with a red tie. His coat is buttoned up almost completely, revealing only a small portion of what's underneath. He wears a mirror with a brown headband. In his right hand, he holds a black suitcase with the words "TOP SECRET" written on it in red letters, like Ron from Accounting. His eyes have slit pupils and shiny black irises, and his mouth is covered by a blue surgical mask. Two large gray antlers sit atop his head. He has dark orange deer markings on his forehead, and his ears are slightly darker than his face. His appearance was changed in "Harlow's Favorite Update." Harlow is the head doctor and essentially the player's handler. He shows up almost every shift and helps with the mechanics:

Unlocks new areas. For example, after the 2nd shift, it unlocks the supply store and the emergency room (rooms 6–8), and after the 3rd, it grants access to the stun gun. Provides performance evaluations. At the end of shifts, he summarizes the results and assigns the player a "performance rating"—this affects progress and access to content. He also warns of risks. On the fourth shift, he reports an anomaly attack and the arrival of an ambulance, and on the sixth, he directly warns: if the hospital loses three patients, the shift will fail. After the 7th shift, Harlow stops actively talking about mechanics, but remains in the lobby - you can talk to him to learn bits of lore.

Ron from accounting

Ron is an anthropomorphic blue lop rabbit (with long ears). He has slicked-back brown hair, glasses, a white long-sleeved shirt, black pants, a tie, and dress shoes. He always carries a briefcase with the words "TOP SECRET" written in red letters. His name tag reads "Ron from Accounting." Who is he and what does he do in the game? Works in the management of Animal Corp. Although listed as an accountant, he knows a lot about the goings-on at the hospital and various anomalies. He gives the player hints and bits of lore. For example, he explains that patients aren't just sick, but "infected," and explains how to deal with certain entities (like the Bedside Monster—maple syrup is useful for that). He's calm and pragmatic, but clearly tired of his job and not particularly loyal to the corporation: he's willing to share secrets, though he doesn't get caught up in the thick of things. He appears on the 2nd or 3rd shift and comes to the hospital as a patient or visitor, gradually revealing details of the plot.

Rattina

Rattina is the twin sister of another character, Ratthew. She worked at Animal Corporation but disappeared without a trace. Ratthew has been searching for her for years, and this story becomes one of the game's key mysteries.

Ratthew

Rattu is a black anthropomorphic rat with a white face, a pink tail, and two protruding front teeth. Ratthew is small in stature and initially acts withdrawn, but quickly opens up and talks about his missing sister, Rattina - she was the smartest in his family and was his twin (minus the pink bow on her head and eyelashes) Ratthew, despite his affection for you, is forced to leave the hospital forever because the authorities are hunting him (due to the fact that he knows too much about government affairs and when he first got into the Accountant's office, he chewed up documents that he should not have had access to)

Barney

Barney is an anthropomorphic rabbit. He has white fur, a brown hat, a bushy black mustache, and brown plaid pants. He appears somewhere between the 2nd and 7th shifts. On the first and second visits he asks for coffee. On the third visit he asks for coffee and a photo. On the fourth visit he asks for coffee and a scalpel. And on his fifth visit, he asks to be hidden in the reception area. In the anomaly version, Barney has a large, toothy grin and large black eyes with white pupils. Barney is a man who loves coffee very much and is an anomaly and a wanted serial killer. If you don't hand Barney over to Officer Duckman, he'll give you the second coffee machine, but if you do, he'll attack the officer, break the camera, and escape (the officer will have to be treated).

Types of anomalies (Anomalies in video surveillance)

Black Eyes: The patient or visitor's face will be covered by a black bar, revealing realistic eyes looking at the player. Unnatural Body: The patient or visitor will appear severely stretched and distorted. Each time the surveillance camera is closed and reopened, their body shape will change. If the player manages to look into the surveillance camera while holding a photograph of the patient, they will also appear stretched and distorted. Most people refer to this as a "boneless" anomaly. Looking at cameras: The patient constantly looks directly at the CCTV camera, even while walking. Emptiness/Darkness: This patient appears completely black. This is sometimes combined with other features. (As of June 22, 2026, if a patient is photographed without other features, they will lose consciousness, indicating their condition.)

"Empty Face": This anomaly appears as a creature with dark, sunken eyes, constantly twitching and standing in a hunched posture. Despite being an anomaly, they are not always hostile. During their recovery phase, they are guaranteed to perform a death ritual, so it's best to stay in the same room with them. Twitching: The patient will experience twitching of both arms, the head, and the neck, causing it to appear longer. The back will also appear hunched. Sometimes this feature is evident without the use of equipment, but more often, this anomaly is only visible through surveillance cameras.

Mismatched Eyes and Sharp Teeth: The patient has a crooked, distorted smile with sharp teeth. The anomaly also has two glowing eyes of different colors, and its voice is guaranteed to be low and distorted. Its head will turn to follow the player. Three Eyes: The patient has three slightly different glowing red eyes, and their voice is guaranteed to be low and distorted. Sometimes the patient's actual eyes are visible. Their head will turn to follow the player. Unnatural Appearance: There are two variants of the abnormal appearance symptom. The patient loses their true face and constantly stares at the player while they move. In both variants, the voice is low and distorted. The first option is a face with large eyes, black pupils, and prominent brow ridges, combined with a wide, painted-on smile. The smile will disappear after a taser shock. The second option consists of small but wide round eyes with black pupils and realistic teeth.

CCTV camera anomalies

Anomalies captured by CCTV cameras are anomalies that can only be seen if the player looks into the camera. The patient will appear normal in the window and on the camera, but not on the CCTV camera.

Photo anomalies

Photo anomalies are anomalies that can only be seen if a player takes a picture of a patient at the window with the camera. All anomalous photographs have the potential to deduct ten sanity points from any player who views them. Leaving the photograph on the table will not reduce the player's sanity.

Anomalies in appearance

Anomalies that can be identified without any equipment. All appearance-related anomalies can also be seen in photographs and on surveillance cameras, but these are not required for identification.

If left unchecked, appearance anomalies usually result in deadly rituals, the appearance of a monster, or the anomaly turning into a werewolf that will attempt to eat your patients and players who get too close.

Anomalies

Anomalies are entities similar to patients. Unlike patients, anomalies exhibit symptoms that can be observed in several ways: through personal contact, through surveillance cameras, and in camera photographs.

One patient may have several anomalies at the same time. If the anomaly is healed, it will say, "I'm hungry," and either leave the hospital without attacking anyone, or attack another patient.

Visitors

In short: regular visitors look like regular anthropomorphic creatures, regular black eyes, don't twitch, sometimes there are individuals with fur coloring in the form of circles or lines (which can be perceived as a rash or something similar), they don't show aggression and behave decently
