Barney is presented as a white creature, a rabbit, with slanted black eyes and a mustache. He wears a brown hat. Top: A classic white structured-collar shirt with subtly rolled-up sleeves, combined with a dark brown V-neck vest with a slight textured or heathered effect. Bottom: High-waisted dress pants in dark gray with a subtle pattern (it looks like a micro-check or tweed). They have darts at the waist for volume and a straight cut that provides structure. Her Anomaly form has a mouth with sharp teeth and large, hollow black eyes with a small white dot in the center, each holding a knife.

As you get to know him, he is kind, considerate, and calculating. Whenever you do him favors, he praises you, calling you "Colleague." Shifts are periods of time during which you must attend to patients admitted to the hospital. Shifts begin by registering patients, who then receive treatment according to their health status. At the end of the first four shifts, Dr. Harlow arrives and unlocks features for you, such as the shuttered door and cameras, the Taser gun, the ER wing, and a shop where you can purchase various items at the start of each shift. Over time, events such as fires, funeral rites, and emergencies are unlocked, bringing an influx of patients, some critically ill and others with abnormalities. Anomalies are creatures that roam the hospital and can attack and kill patients, visitors, and players. Your mission is to keep the hospital safe, the patients alive, and minimize the presence of anomalies. You can repel them using your stun gun (unlocked during Turn 4) and the pistol, which you can buy from the shop. You can use these items before an anomaly attacks or during an attack.

Classes are modifiers that can grant specific starting stats, items, or additional utility and function. You begin the game with the free Intern class. As you play and earn Animal Coins, you can purchase more classes. Some available classes include Nurse, Secretary, Surgeon, and Doctor. Upon purchase, classes start at Level 1 and can be upgraded with experience gained from playing, which will improve the class's perks.
