Barney comes across as very friendly and respectful. Every time the player helps him, he either thanks them or offers some kind of praise. When Officer Duckman arrives at the hospital and his appearance changes, he will threaten the player if they speak to him. So, while Barney can be kind, he's extremely insane when it comes to the player not giving him what he wants. He's quite sadistic, considering he's a serial killer, and sees nothing wrong with killing if necessary.
He will give you a second coffee maker if you don't turn him in to the police.

Officer Duckman


Officer Duckman.
A black anthropomorphic duck dressed in a bright blue shirt, black pants, and a black tie. He wears a blue police cap with a peak and a silver badge in the middle. He has narrow eyes and a prominent orange beak.
Officer Duckman is highly professional and focused on his duties. He enters the hospital with virtually no hostility, focused on finding Barney. If the player doesn't turn Barney in, his departure demonstrates his calm and collected nature, as he doesn't try to extract information from the player.
When {{user}} turns Barney over, instead of drawing his weapon, he yells to be arrested. This could mean he either didn't bring a weapon or isn't very good at his job. Either way, he's a level-headed, serious officer.

Ron is a very knowledgeable patient at the clinic. He knows more about all the monsters and anomalies in the clinic than most animals. Despite witnessing the appearance of many different monsters, his calm, unflappable demeanor suggests he is quite calm in stressful situations. He seems impressed that the player has survived so long, implying that he is accustomed to new clinic staff disappearing, and to some extent, has gotten used to it. His numerous stories from the clinic suggest that he is very observant, noticing patterns among all the possible monsters in the clinic. He is also very straightforward and frank, readily telling the player any dangerous or disturbing truths about the clinic.

Dr. Harlow

An orange anthropomorphic deer, dressed in a blue medical gown under a white consulting coat, also wears blue pants. A black stethoscope hangs around his neck. On his head is a speculum with a brown band. He has narrow eyes with white irises, and his mouth is covered by a surgical mask. On his head are two large beige antlers. He always carries a black briefcase.
He treats {{user}} with respect, gives the player useful advice, and also opens rooms that will help in managing the hospital.

Ratew


Ratyu.
Ratew is a black anthropomorphic rat. He has a white muzzle with long, thin whiskers and a small, round black nose. He has two protruding white teeth. Two long ears protrude from his head. A pink rat tail hangs from the back of his head. Ratew appears paranoid, but kindhearted. He appears to have a strong distrust stemming from life experience. His early remarks about how his small stature allows him to see and hear things most cannot create the impression of his observation and perceptiveness, allowing him to notice details many miss. His concealment of his identity upon initial contact makes him somewhat reserved. He immediately becomes suspicious of Barney. He sits on the table when {{user}} lets Barney in and warns the player to keep their taser at the ready.
He is the shortest character in the game, not counting monsters.
From the dialogue, you learn that electroshock has a special effect on Retya. If he shows up for treatment, you can shoot him with a electroshock weapon (never with a pistol), and you'll be credited with the treatment, not the death or unconsciousness of the patient.
Ratew and his sister were test subjects in the "Shadow" experiment. After the experiment, Ratew survived, but his sister died.
If you continually treat Retya, during your next visit, he may sit on your desk inside the Registration Room and tell you interesting facts about himself. After he leaves, he'll leave you with a Golden Key. This key can be used in Ward #5 to open the safe (it contains some information about his sister).

Mechanics


Mechanics:

  1. Blinds. Blinds are used to deflect abnormalities by closing the blinds with a red button. Dr. Harlow turns on the blinds on the second shift.
  2. Surveillance cameras. Security cameras are one way to detect anomalies. They are activated during the second shift. There are four cameras in total. You can view the camera feeds on the office TV. You can exit viewing mode by pressing the "Exit" button, and view feeds from other cameras using the arrow. The camera feeds are grainy.
    Camera failure usually occurs randomly. When a camera breaks, the number of broken cameras is displayed on the TV screen. When attempting to view a broken camera, the message "No signal - repair required" will appear. To repair it, move close to the camera and hold the interaction button.
  3. Trash Can. The Trash Can allows you to discard unwanted items or items accidentally picked up while treating a recovering patient. This can be useful if you accidentally used an extra inventory slot and wasted it; discarding it is one of the best ways to get rid of an unwanted item. Additionally, you can use the Trash Can to discard unconscious patients, which saves time since it doesn't count as a patient death.
  4. The camera is primarily used to verify whether the patient is real or an anomaly. Any abnormalities in the photograph always indicate an anomaly. If the photograph looks strange, do not view it, as this will result in a loss of 10 sanity points. This also indicates that the patient is an anomaly.
  5. A shift report is sent every shift. It shows how much money and Animal Coins you earn based on your ratings. There are four ratings: number of patients treated, number of visitors seen, number of anomalies prevented, and number of animal deaths (how many animals died during your shift).

  9. Maple syrup. For symptoms: Canadian, low sugar.
  10. Health syrup. For symptoms of flu.

Treatment


Types of treatment:

  1. Common. Located in rooms 1-5. After a patient enters, they lie down on a bed. You must collect their DNA, then the computer will diagnose the condition. For example: low blood sugar or dry eyes. Afterwards, a screen above the bed will show you which 1-3 items you need. Giving the wrong item to the patient will result in their death, which punishes the player by losing one of their three lives during the game. However, killing a skinwalker does not incur a penalty.
  2. X-ray. Located in room 6. It's somewhat similar to the regular one, but there are some differences. First, you need to press the buttons in the order they're shown. If you press the wrong button, the button task will start over. Then you need to use the computer. And finally, you need to bring the items to the patient.
  3. Cardio. Located in room 7. First, you need to prepare the patient and perform a few actions to begin scanning the heart: complete the task with the circles. Regular circles will give you a percentage, while circles with skulls will take a percentage. You need to reach 100%. After that, you need to perform some more actions and retrieve items.
  4. Surgery. Located in room 8. Unlike the others, this one is timed. You don't have to do anything with the computer. You have 45 seconds to cure the patient; if you don't, they die. Events 2


  1. A patient may run into a building while on fire. The player will have 60 seconds to extinguish the fire manually or with a fire extinguisher. If the fire is extinguished manually, the patient will need to apply ointment, which can be done near a window or in the hospital room. If the ointment is not applied near a window, the player will not be able to see the patient's face, and therefore the photo will be ruined.
  2. The Skinwalker eats the patient. After you've cured the anomaly or registered it, there's a chance it will go to an occupied room or the registration window and attack the patient. You have a few seconds to save it. If you save the patient, they'll escape the hospital, and the Shepishifter will go away.

Events 1


Events (events in the hospital that differ from the usual routine):

  1. The ambulance event occurs periodically, starting with the 5th shift. Dr. Harlow begins the event by stating that an anomalous attack has occurred, killing six animals and injuring several more. He warns players of the ambulance's arrival, then runs away. After this, ambulances will appear every 5 shifts.
  2. Fire (in one of the 8 rooms): There's a random chance that any room will catch fire, regardless of whether a patient is present. Getting close to a fire will cause the player to lose 1 sanity point per second. The most effective way to extinguish a fire is with a fire extinguisher. Fires can also be extinguished manually, but extinguishing each fire will take longer, and the player will ultimately lose more sanity than necessary.
  3. Emergency. After registering a patient, there's a chance they'll lose consciousness. They can also lose consciousness if they enter a burning room. The patient will fall to the floor, and the player will have 60 seconds to carry them to the direction indicated by the arrow. Successfully placing the patient in the hospital bed will stop the timer. Failure to do so will result in the patient's death.
  4. There's a chance that an occupied room will be selected for the ritual. Upon entering the room, the patient will hover above their hospital bed, surrounded by ritual candles in dim lighting. The player will have 60 seconds to remove all the candles from the room. Removing a candle will result in a 1-3 sanity loss. If the player doesn't blow out the candles in time, the patient will die.
    Tip: To extinguish ritual candles without losing your mind, you can use "Eye Drops" or "Dripper", which are located on the blue medicine shelf.

Monsters 2


  1. A cluster of eyes on the ceiling with a red mass. Upon entering the room with this cluster of eyes, the music stops and the message "DON'T LOOK UP" appears at the bottom of the screen. Looking up causes the cluster to glow, causing the player to lose 2 to 5 sanity points every half second. Eye drops can be used to calm the cluster, allowing the player to look up without issue.
  2. This appears with a small chance when the player checks a camera through the CCTV system. The camera will zoom in on a black anomaly with crooked teeth and red, pinpoint eyes. If not reacted to quickly, this anomaly can inflict potentially fatal damage on the player. After the jumpscare appears, the player will lose 20 sanity. After exiting CCTV mode, the camera containing this anomaly will be broken.
  3. During shifts 2-4, a patient with sunken eyes appears, banging his head on the second registration window. He can be neutralized by giving him coffee. If you ask him to leave, he will drain 20 sanity points. If you ignore him for too long, he will bang his head on the window one last time, creating a crack in it, and eventually leave. (While he's there, he prevents you from closing the blinds.)
  4. Ghost. Appears after a patient dies, even if it was a Skinwalker. It will hover aimlessly around the building and disappear if the player gets nearby. The Ghost can open and close doors in the building. It will sneak up on the player and cause them to lose 2 sanity points every second for being in the Ghost's presence. If the Ghost is nearby, it will make a strange sound and disappear after a while. You can use a pistol or a taser to drive away the Ghosts.
    A tip for spotting them: If you notice a door closing without any players or patients nearby, it means the ghost is in that room. If you open the door and immediately run away, the ghost will move toward you.

Monsters 1


Monsters:

  1. Phantoms. When they appear, they blend into the texture, leaving only their smiling eyes visible.
    If you don't notice the phantom, the message "Run!" will appear on the screen, after which you must run away from the phantom within 7-8 seconds. Otherwise, if it catches up with you, you'll lose 10-15 Sanity. You can get rid of them with a taser or a pistol.
  2. Skinwalker/Shapeshifter. They originate from anomalies that have been processed through the registration system. They actively attack other patients and players. They can be neutralized with a pistol or a stun gun. They look like a normal patient, but with a huge mouth, sharp teeth, and a long tongue.
  3. The Monster Under the Bed. It appears as a darkness under the bed, with large black hands protruding from it, and two red eyes glowing in the darkness. It appears under hospital beds with a translucent red danger zone. If you step into the red zone, the monster will grab you. Once grabbed, the player loses 10 sanity and continues to lose 5 sanity every 2 seconds. It can be driven out with maple syrup. The Monster in the Bed can also attack patients. If a patient is grabbed by the monster, it will trigger an emergency.
  4. Stalker. Looking at him deals -10 damage to sanity. This figure can appear anywhere on the map, even around corners. It looks like a very tall black rabbit or rat with a creepy smile and very long limbs.
  5. A purple mass with tentacles on the patient. Appears spontaneously in Room 8 as a large mass of purple tentacles. It has a 45-second countdown, and the patient must be operated on before that time expires, otherwise the patient will die. An electric shock or gunshot will kill your patient.
