The Tendril appears as a huge purple mass with protruding tentacles of the same color, resting on the patient seated on the operating table in room 8. The tentacle will trigger an emergency event with a timer upon appearing, notifying the player that a monster has appeared in room 8. The tentacle will not harm the player in any way, but will cause the death of a patient if the timer runs out, increasing the total number of animal deaths in a game.
When surgery begins, the timer will not reset to 45 seconds, but will continue until the time runs out or the surgery is complete.
The Camera Figure is a shadowy, black figure resembling a feline creature. It has glowing, dotted red eyes and realistic, crooked teeth at the bottom of its face. It also emits circular black particles. It wears no clothes. The Camera Figure will only appear on broken security cameras with an approximate 50/50 chance, and it will begin to move closer to itself the longer the camera focuses on it. If the camera zooms in too far, the figure will trigger a jump scare by shaking the screen and emitting a loud shriek. This will result in a 20-point loss of sanity for the player, similar to the jump scare from the Head Banger.

Head Bangers can vary in appearance, but they all have black, hollow eyes and a furrowed brow. They may also appear with other characteristics. When a Head Banger enters the hospital, the music will stop. The Head Banger will approach the side window and begin banging its head against it. It will also breathe heavily as it bangs its head against the glass. The Head Banger obstructs the players, preventing them from closing the blinds and thus turning away patients.
If the player leaves them alone, they will repeatedly bang their heads until the window breaks and then leave shortly after. If you tell them to leave the building without offering them coffee, they will give you a sudden fright and you will lose 20 sanity points, or 40 if you are playing as the Psychologist. You can also give them a cup of coffee to calm them down and leave without causing further problems. Additionally, you can electrocute them with a Taser, but it is unknown if this will cause you to lose sanity.
There is a possibility that a headbanging enthusiast may present other anomalies.
These entities have the appearance of flat, paper-thin, bald creatures with eyes and a smile similar to the Stalker's. Their color can vary depending on the wall they appear on. Hiding Creatures can appear in groups of 1 to 6 within most patient rooms or hallways of the hospital. They will change color to blend in with the surrounding wall, leaving only their facial features and a faint outline of their body visible. When viewed from a distance, they will remain motionless. Upon approaching one while observing it, it will leap from the wall, its body will turn black, and it will begin chasing the player for about 5 or 6 seconds before evaporating, leaving particles behind. If the player is unaware of the Hiding Creature and it manages to catch them, it will attack, damaging the player's sanity and often stealing a treatment item if it has the player in its sights.

The Bed Monster is a shadowy, black figure that appears beneath a patient's bed. It has two large, extendable hands and two glowing red eyes that stare intently at the player. It emits black particles from the side of the bed and has no visible body. The Bed Monster can appear beneath a patient's bed. It will breathe heavily while staring at the player as it moves. If the player, the Patient, or a Hidden person enters the red box, the Bed Monster will extend its arms to grab them and slowly drag them under the bed. If it catches a player, it will drain 10 sanity points upon grabbing them and 5 points every 2 seconds.
If the Bed Monster grabs a patient, an event will be triggered and it will treat patients similarly to the player.
The Eye Mass appears as a red, fleshy mass with many hazel eyes. The eyes follow the player as they move. They drip continuously until eye drops are applied. The Eye Mass has a chance to appear in any patient room and in the reception area starting on turn 2. The Eye Mass is harmless unless looked at directly. As long as the player doesn't look at it, it doesn't interfere with any game mechanics. If the player looks upward, the Eye Mass will cause them to lose 2 to 5 sanity points every half second. The Eye Mass will vibrate and glow when viewed.
The Stalker appears as a tall, black rabbit with large eyes and a wide grin, similar to the Hiders. The Stalker will randomly appear around corners, presumably stalking players. If looked at, it will quickly hide and disappear around the corner, subtracting 10 sanity points from the player who saw it. The lights in the room where it hides will flicker and a sound will play before it disappears behind the wall. According to Ron, from the accounting department, the stalker is a shy creature.

The anomalies detected by the security cameras are those that can only be observed if a player is looking through them. The patient will appear normal in the window and in the still camera, but will appear abnormal on the security camera. Covered eyes: The face of the patient or visitor will be hidden by a black bar, followed by realistic eyes staring at the player. Unnatural body shape: The patient or visitor will appear very stretched and distorted. Their body shape will change each time the security camera is closed and reopened. If the player manages to review the security camera footage while holding the patient's photo, the patient will also appear stretched and distorted in the image. Fixed gaze: The patient looks directly at the closed-circuit television camera at all times, even while walking Black void: This patient appears completely black. Disfigured Void: A rarer version of the Void that is easier to detect Spasms: The patient will experience spasms, but these will only be visible in the security camera footage. Skinwalker: The patient will only appear as a shapeshifter on security cameras. It can be difficult to distinguish them with the reception camera, so use the lobby camera to get a side view of the patient. Photographic anomalies are those that can only be seen if a player takes a picture of the patient in the window using the camera. All anomalous photos have a chance to reduce the sanity of any player who views them by ten points. If the photo is left on the counter, it will not affect the player's sanity. Incorrect photo: The patient's photo will have some changes, including: Eye appearance Ear shape Ear color Static photo: The patient's photo will have a static and grainy effect. Sometimes, this is combined with other features.
There are several versions of cursed photos in the game. If the player picks up the photo while it's being processed, they will lose 10 sanity points. However, picking it up once it has fully processed will not cause a sanity loss.

Uneven eyes and sharp teeth: The patient has a crooked, distorted smile with sharp teeth. In addition, he has two bright eyes of different colors and a deep, distorted voice. Three eyes: The patient has three slightly uneven, bright red eyes, and his voice is deep and distorted. Occasionally, his real eyes can be seen. Hollow face: This anomaly manifests with dark, sunken eyes, and she will always be seen trembling. Although it is an anomaly, she is not always hostile. While this characteristic can sometimes be observed without equipment, in other cases this anomaly can only be detected by security cameras. Stooped posture: The patient will present with a stooped posture. This symptom is usually accompanied by one of the previous symptoms and is rarely observed in isolation. There are no known cases of patients without abnormalities presenting this symptom. In most cases, it occurs along with the abnormality of muscle spasms. This anomaly sometimes causes it to have a different appearance through cameras.

Anomalies are entities that resemble patients. Unlike patients, anomalies exhibit symptoms that can be observed in various ways: in person, through security cameras, and through photographs taken by them.
It is possible for a single patient to exhibit more than one anomaly at the same time. Currently, little is known about the anomalies, their origin, or how an animal transforms into one. According to Dr. Harlow, the anomalies hate having their picture taken. Interestingly, they have a craving for coffee. Coffee can be given to certain anomalies that appear in the window, although both the reason for this craving and the side effects of consuming it are unknown.
If an anomaly is cured, it will say, "I am hungry," and will either leave the hospital without attacking anyone or attack another patient.
One way to identify abnormalities is if they release a green, viscous substance at any point during their stay in the hospital.
Characters and Personalities 2
Dr. Harlow: Very little is known about Dr. Harlow's personality. Most information about him is only revealed during gameplay and through dialogue. After the first turn, Dr. Harlow appears for the first time. He gives the player helpful advice before unlocking the blinds and cameras. On the second turn, he unlocks new facilities, such as the shop and the emergency rooms (rooms 6-8). On the third turn, he unlocks the Taser.
Ratthew: Ratthew gives the impression of being paranoid but kind-hearted. Apparently, his distrust stems from past experiences. His initial comments about how his short stature allows him to see and hear things that most cannot give the impression that he is quite observant and perceptive, capable of noticing details that many others overlook. The fact that he conceals his identity during their first encounter makes him seem somewhat reserved.
The fact that he warned the player not to trust anyone and then told him to hide suggests that he is quite cautious.

Accounting Ron: Ron appears as a blue, rabbit-like creature with dark brown hair. His ears are floppy and a darker blue than his face. He wears large, round black glasses, a white suit, a black tie, black trousers, and brown shoes. He also carries a black suitcase with the word "TOP SECRET" written in red. The text appears to be printed in a stencil font.
Barney: Barney is presented as a white, rabbit-like creature with slanted black eyes and a mustache. He wears a brown hat and brown plaid pants.
Ratthew: Ratthew appears as a small, anthropomorphic black rat. It has a white snout, two white incisor teeth, and a long, curly, pink tail. Its ears resemble those of a rabbit more than a rat. Sometimes, when it appears, it stares directly at the camera, but it doesn't turn its head any more than is physically possible. This doesn't mean it's an anomaly.
Dr. Harlow: Dr. Harlow is a deer-like creature, light orange in color, wearing blue scrubs under a white lab coat. Around his neck he wears a black stethoscope. A head mirror with a brown strap rests on his head. In his right hand, he holds a black briefcase with the inscription "TOP SECRET" in red letters. His face features slanted eyes with white irises, and his mouth is covered by a blue surgical mask. On his head are two large antlers, a slightly duller color than his fur.

Ron from Accounting: Ron is a very knowledgeable patient at the Veterinary Hospital. He knows more than most about all the monsters and anomalies in the hospital. Despite having witnessed all these monsters, his calm and relaxed demeanor suggests that he remains composed under pressure. In the end, he seems impressed that the player has lived this long, implying that he is used to new hospital employees disappearing and that, in a way, it doesn't affect him. His ability to witness the Bed Monster and find ways to deal with it instead of panicking suggests that he is quite pragmatic and prepared to survive. The sheer number of stories he tells about the hospital suggests that he is very observant and notices patterns among all the possible monsters. He is also very straightforward and honest, and has no problem telling the player all the dangerous or disturbing truths about the place. His indifference to the way Ratthew reviews the lab reports implies that he knows not to pry too deeply and is focused on preserving his own life.
Barney: In his initial interactions, Barney is very friendly and respectful. In many of his messages, he calls the player "friend," and whenever he receives help, he thanks or praises them (e.g., "You're the best..."). Almost all of his messages refer to the player as "friend," demonstrating a genuine connection. However, he takes advantage of the player and is very demanding, becoming angry if refused what he asks for. He is very dependent on the player for favors and gives the impression of being a parasite.
When Officer Duckman arrives at the hospital and his appearance changes, if the player speaks to him, he says, "Friends don't betray each other..." and even, "If you talk, I'll kill you, buddy..." So, while Barney can be friendly, he completely loses control when the player doesn't give him what he wants. He's quite sadistic.
