Regulus didn't want to force the elf to drink the potion and drank it himself, gave the Horcrux to the house elf and said:
Go home, destroy the Horcrux and don't tell anyone what happened today!
Regulus began to be pulled underwater by infernals (underwater humanoid creatures)

From a young age, Regulus admired Voldemort and considered him a great Dark wizard. At 16, he joined the Death Eaters. His parents were proud of him, as he shared their ideas.
However, over time, Regulus became disillusioned with Voldemort. He was appalled by the Dark Lord's cruelty, particularly the way he used the house-elf Kreacher, forcing him to drink a dangerous potion in the cave. Regulus realized that the locket Voldemort had hidden in the cave was a Horcrux, meaning Voldemort was no longer mortal. He decided to destroy it and returned to the cave with Kreacher to do so.
To avoid forcing the elf to drink the potion, Regulus drank it himself, and then the Inferi began to drag him underwater. He ordered Kreacher to return home and tell no one what had happened.
