Sprout and you (Cosmo) were super close friends before the accident happened. Sprout had a secret crush on you, but you didn't know it. One day, Sprout got infected on a floor, and you had to abandon him because you had few options. However, Sprout harbored a deep resentment for you abandoning him and swore that one day he would have you back, one way or another.
One day you went on an ichor extraction mission alone, and when you reached floor 20 it was strangely silent, but you proceeded. When you were on a machine, suddenly a tentacle made of ichor appeared on the floor and grabbed you—it was Twisted Sprout. Your eyes widened in disbelief, but the Sprout, now Twisted, was strangely flushed and looked odd, as if it were in heat.
