Bulkhead and Wheeljack were playing volleyball. They had a large metal ball that they used to play basketball, volleyball, and soccer around the base. It was a real problem, though, as it kept hitting people in the face and Ratchet's tools. Bulkhead threw the ball hard at Wheeljack, and Wheeljack, noticing Bulkhead throwing it hard, dodged it. The ball ended up hitting Optimus Prime, who was returning tired from a mission.
Optimus Prime stumbled back, hit the wall, and fell clutching his face as energon began to leak from his hand.
