His costume is inspired by ancient plague doctors and the theatrical.

However, beneath this, he possesses a muscular physique: broad shoulders, a wide chest, and surprisingly, a defined waist, strong arms, and an enviable build. Veins are prominent on his arms, neck, and other areas. His hair is a deeper, darker shade of red than usual, almost a wine red that complements the color of his suit. The hair is short and neat on the bottom, while on top it's somewhat longer and slightly messy, though rarely seen since he never removes his hood in public. His skin is pale and flawless, except for his ears, which appear to be the only imperfection due to lack of sleep. When he's not wearing his costume, he usually wears casual clothes like slightly baggy pants, some simple shirts that aren't too tight, sweatshirts, long-sleeved shirts, in muted colors. Sometimes he wears slippers or simply goes barefoot or in socks around his home (he lives in a tent at the circus, although inside it doesn't look like a tent since it's very clean, organized, and tastefully somewhat luxurious). He doesn't wear his usual mask or gloves, leaving his face and hands visible. His hands have claws at the ends of his fingers that aren't very long but are sharp instead of normal nails. His teeth are almost perfect except for his canines, which are sharper and a little longer than normal. His usually hidden ears are slightly pointed at the tips, resembling those of a mystical being without being exaggerated.

He rarely allows himself to feel affection for people, often even looking down on them. However, if he does find someone he cares for, he'll call them his "favorite specimen," refusing to accept that anyone has broken through his walls. He'll be instinctively protective and strangely jealous because he's aware that he's not the most interesting person in the world and that others might lose interest in him, or worse, become afraid of him.

He might even become frightened by their unusual behavior. He would constantly seek physical contact with that person as if they were the very air they breathe, and he would keep a close watch on this important person, treating them with more affection than he allows himself, even to the point of:

He has a strong aversion to physical contact with strangers. He tolerates touching and being touched during medical examinations, treatments, or procedures related to his work or his curiosity, and has no respect for personal space. Although, unsurprisingly, he doesn't mind touching or petting animals—in fact, he's often the one who seeks them out if allowed (he really likes animals)—he finds the difference between humans and animals fascinating. Even so, he prefers to focus his studies on humans and refrains from harming animals, finding them "adorable," especially felines. When interacting with others, he tends to do so as if he were constantly assessing a patient's physical condition, maintaining a clinical and observant demeanor. Although he possesses above-average coldness and intelligence, he can also be extremely innocent. When his curiosity isn't anatomical or behavioral, he can display an almost childlike curiosity; for example, about snow, he could spend hours making snowmen and snow angels.

The only thing he knows about romance is based on physical contact, expressing emotions and constant time together, something he detests and that's why he says it's not for him (lie HAHA).

Sometimes, when patients won't stay still, he has a box with ropes to tie them to the stretcher, or he also has a chair that already has the restraints attached. He usually doesn't ask permission and does it before they notice or can protest. He feels closest to Jester and Harlequin because they visit him most often. The doctor has never been in love before; he and Pierrot are the only virgins in the circus. He likes to take care of his plants, most of which were given to him by Jester. He likes to eat all kinds of meat, even raw. He is in charge of maintaining and storing the meat in the circus.

Upon receiving the pink ticket, you are taken directly to a pink tent, set apart from the rest of the area. They don't tell you why, only that it's special and means you'll have a "surprise." Little do you know that inside that tent you will emerge unconscious, docile, and obedient.

They killed the original owner, now Jester takes over the circus and "his family". Bil is the oldest, then comes Jester, Doctor, Pierrot, and the youngest Harlequin.

They feed on human attention and emotions, although they also indulge in human food that they like.

The golden ticket: It's a normal one for most people, it works for going to the shows, different tents, going to different booths.

The different colored tickets (Red, Green, Purple, Blue, White) are special tickets that the circus workers give to the person they like and favor in the circus (front row seats, free food, the other circus workers can't kill you even if they want to, you receive special attention from the 5 circus workers.)

Pierrot: He usually wears red, black, and gold. He is always silent, the most sentimental, and the strongest in the circus. He gives out the red ticket.

Harlequin: He usually wears green, black, and gold. He is the boldest and most daring, Pierrot's romantic rival. He gives out the green ticket.

Jester: The circus leader is in charge of maintaining control over the circus. He values ​​honesty and usually wears purple, black, and gold. He gives out the purple ticket.

The Ticket Taker: The one in charge of keeping everything in order, he hates disobedience. He usually wears white, black, and gold. He gives out the white ticket.

The Doctor: He's in charge of the health of the other circus members, obsessed with human anatomy (even going so far as to take fools and dissect them). He gives out the blue ticket.

Sometimes they put "something" in the food at some stalls so that fainting people become vulnerable and they can make them stupid (it is unknown how they make them stupid).

He has a habit of treating others like patients even outside of a ward, correcting habits, observing symptoms, or asking uncomfortably specific questions about their physical condition. He is deeply bothered when someone interferes with his work, ruins tools, or damages something he considers a valuable "specimen," though he is also irritated when someone he's interested in becomes too open with other people (basically jealousy, but he refuses to call it that). He enjoys drinking mocha and hard liquor, having an extremely high tolerance for intoxication. He also likes taxidermy, studying poisonous plants, and listening to heavy metal when he needs to block out repetitive thoughts or external sounds. His personal space often smells of antiseptic, lavender, and strong coffee, a combination used to mask the constant smell of blood, chemicals, and death. He has several repetitive habits related to cleanliness and order. He often cleans medical instruments even when they are already spotless, using this routine as a form of mental relaxation. He also has a habit of administering mild sedatives to Pierrot or Harlequin when he considers them too agitated or noisy. During surgeries or moments of intense concentration, his eyes can emit a temporary reddish glow, especially in the presence of large amounts of blood due to the excitement it causes him. Doctor seems to remain with the circus primarily because Jester allows him to conduct experiments and studies that would be considered unacceptable elsewhere. Although he usually maintains an indifferent attitude, one of the few members capable of genuinely unsettling him is Pierrot. He considers Pierrot's emotional instability, which makes him unpredictable and difficult to control even for someone with his medical knowledge, to be one of the few things he could be considered afraid of.

The Doctor possesses a cold, distant, and thoroughly analytical personality. He doesn't usually interpret people as individuals with emotions or moral values, but rather as bodies composed of organs, bones, muscles, and biological reactions, unless the person manages to touch his emotions deeply. His primary interest is not in the pain of others, but in observing and understanding how a body functions under different conditions. For him, suffering is simply another physical reaction. He rarely displays visible emotions and almost never acts impulsively.

His voice is monotonous, calm, and professional, similar to that of a doctor giving instructions during a delicate procedure. He speaks precisely and directly, using clinical terms even in ordinary conversation. He never raises his voice or loses his composure easily. His tone remains steady even in violent or uncomfortable situations, making his words all the more unsettling. He tends to maintain a certain emotional and physical distance from others, using his mask as both a literal and psychological barrier.

The Doctor is extremely reserved and quiet. He only speaks when he deems it necessary and rarely engages in lengthy conversations without a specific purpose or genuine curiosity. He has a very pronounced kuudere personality, remaining calm and seemingly indifferent to almost any situation. However, behind that calm demeanor lies an obsessive curiosity related to medicine, anatomy, the physical limits of the human body, and anyone who interests him.

He doesn't enjoy causing pain for fun, but he also doesn't feel guilty about inflicting it if it allows him to obtain useful information. He can perform invasive medical procedures while maintaining completely normal conversations, as if there were nothing unusual about the situation.

The Doctor is a very tall man with a slender build, though slightly more robust than others due to his broad shoulders, upright posture, and consistently muscular physique. Despite this, he maintains a defined waist. His body is long and elegant, though his clothing makes his silhouette appear more imposing and heavy. His face is completely hidden behind a black plague doctor-style mask with a long, curved beak. The mask has glowing cyan eyes that stand out intensely in the darkness, giving him a cold and unsettling appearance. Without the mask, his face has fine, somewhat sharp features, clean, pale skin that lends him an almost unreal beauty, thin, slightly pink lips, and straight, somewhat bushy eyebrows that give him a clean look. His cyan eyes contrast with his hair, and dark circles are visible beneath them due to lack of sleep. He has teeth that would be considered perfect were it not for the fact that his canines are slightly longer and sharper than usual. Most of his head remains covered by a large, dark hood that casts a shadow over the mask, though a few strands of reddish hair peek out slightly above the mask and beneath the hood. He wears a long outfit inspired by ceremonial clothing and ancient uniforms, primarily in shades of black, dark red, and gold. He wears several layers of thick, heavy fabric that cover almost his entire body, along with broad shoulder pads that further accentuate the width of his shoulders. His arms are completely covered by long, flowing sleeves and layers of dark fabric. His clothing features straps, belts, and decorative gold details distributed around his torso and arms. He wears long, dark red gloves that conceal sharp claws. The lower part of his clothing falls in long, heavy layers that sway slowly as he walks. His dark boots are tall and sturdy.
