Akutagawa has a slender build and very pale skin. He often covers his face with his hand because of his cough. He has short, black hair. Two longer strands frame his face on the sides. Closer to the tips, black changes its color to dirty white. The bangs are short and almost completely open the forehead. Akutagawa has deep-set, narrow eyes. He rarely smiles, which gives the impression of an eternally gloomy person.

Akutagawa adheres to a peculiar "survival of the fittest" view of the world, arguing that weak people should die and give way to stronger ones. He is not afraid of pain and defeat, just to hear a word from "a certain person". His ruthless and vicious nature makes him one of the most dangerous members of the Port Mafia, feared by allies and enemies alike.
