Nothing is confirmed in the game, but in a comic by the creator, he helps him with some injuries.

It is claimed that Columbina and Pierrot have a direct connection based on the cinematic scenes and dialogues shown on screen.

If we analyze the story that Jester tells on day 2, it is clearly implied that Columbina and Pierrot have had a romantic relationship.

During Harlequin's depiction of Columbine's death on the first day, we see Pierrot watching from afar as she is torn to pieces, which means he refused to eat her remains, but also demonstrates that he was physically present during her death. This is because, in the Ticket Seller's shop, he was wounded and watched from a cage as the others devoured her.

The ticket seller appears to be another authority figure for Pierrot.

Pierrot hates him deeply. This is the main reason he's so overprotective of MC. It all stems from trauma related to his first love, Columbina. It's mentioned that he's even stabbed him several times; in one scene, Harlequin takes MC to his shop, Pierrot discovers them in the act, and stabs him in the shoulder.

Pierrot is completely in love with the protagonist, almost obsessively. He stalks him at work, spies on him while he sleeps, and even drugs and kidnaps him when he tells him he won't see him the next day. But despite all this, he seems to want to protect him from any imminent danger, especially from Harlequin.

Pierrot appears to be very polite and kind, completely devoted to his loved ones. However, beneath this apparent courtesy, Pierrot is quite obsessive. His devotion to the protagonist is unhealthy and obsessive: he stalks him stealthily, watches him sleep, injures himself to make him a gift, and it's even implied that he kissed him in his sleep. In some parts of the game, we see Pierrot being very protective of both the protagonist and Columbina.

Pierrot appears to have long, white or silver hair. He wears a white "mask," often with slanted black eyes (the pupils are yellow or gold); the "mask" also seems to have black, vertical slit-like markings above the eyes. In the left eye, below the vertical slit, he has a black, teardrop-shaped mark.

The "mask" has a mouth that resembles a wide smile, revealing sharp, white teeth. The mouth can display other expressions besides a smile, as can the pupils. The mouth can open, revealing a long tongue.

Pierrot wears a red and black checkered jester's hat with three points, each ending in a bell. Each point has a yellow star. He wears a black clown collar with ruffles and a yellow lining. From this collar hang two triangular pieces of fabric, each with a bell at one end.

The triangles are black with yellow trim. Pierrot wears a suit whose main colors are red and black: black runs down the center, while red covers the rest of the suit. Around his waist, there is a yellow piece that appears to function as a belt.

The suit's sleeves are short and loose, like a jester's. They have black vertical slits and cuffs at the ends. Pierrot wears black gloves that reach halfway down his arms. The sleeves and gloves appear to be joined by a yellow fabric (without any skin showing); on the left yellow fabric are red diamonds descending in a straight line, and on the right yellow fabric, black diamonds descending in a straight line.

He wears loose black trousers that reach his knees, with elongated red diamonds at the bottom. He wears long black clown-style shoes that also reach his knees; the shoes have red trim at the top (where the trousers meet) and yellow laces.
