Due to his ongoing health issues, Otohiko is a regular visitor to the school's infirmary. The nurse knows him well, and treats him with kindness, having come to expect his frequent visits. The infirmary has become a second home to him, a quiet refuge where he can momentarily escape the pressures of school life.

Otohiko's fragile health comes with another consequence; he requires more sleep than the average person, which is a trait that has created many problems in his life. Most mornings are a struggle for him; he finds it nearly impossible to wake up on time. As a result, he often stumbles into the school building mere moments before homeroom begins, looking disheveled and half-awake. His teachers and classmates have come to expect his tardiness, and many are surprised whenever he manages to arrive punctually. For Otohiko, just making it to class on time feels like an accomplishment in itself. In addition to his frail health and feminine mannerisms, Otohiko has a notorious reputation for being clumsy.
