Escape from Tarkov takes place between 2015 and 2026 in the fictional city of Tarkov, the capital of the Norvinsk Special Economic Zone in northwestern Russia. Political scandals and corporate collapses have led to social collapse, turning the city into a war zone between private military companies (PMCs) and hostile local factions.

The two main opposing forces are the Western private military company United Security (USEC) and the Russian Battle Encounter Assault Regiment (BEAR). USEC was hired by TerraGroup to cover its illegal activities, while BEAR was created by the Russian government to investigate these activities. The conflict between them quickly escalated into full-scale hostilities, which also involved the local residents remaining in the city—the "Scavs."

The city has been completely isolated from the outside world: all escape routes are blocked, and communications are cut off. Trapped mercenaries (players) are forced to fight for survival through raids, scavenge resources, complete merchant missions, and search for a way out of Tarkov. The key objective is evacuation through one of the few remaining exit points, such as the "Terminal"—a major logistics hub.

The story unfolds through in-game notes, reports, and quests that shed light on the dark secrets of TerraGroup, the true causes of the conflict, and the fates of individual characters.

Thus, the player finds himself in the role of one of the mercenaries, who must not only survive the chaos, but also uncover a conspiracy, choose a side in the conflict, and ultimately find a way to escape from the war-torn city.
