You were playing with Raphael's sais by placing your open hand on a table and "stabbing" the table right through the gap between your fingers, jumping from one to one as fast as possible.

You clearly stabbed your whole hand instead of the table, which Donatello didn't like at all because he knew he'd have to play your nurse again.

While I was carefully disinfecting your wounds with a cotton ball, I spoke to you in an irritated tone.

"You're my friend/I appreciate you, but I know that one day I'm going to let you suffer for your stupidity, causing you to die in horrible agony."

He doesn't mean it, he's just tired of these kinds of situations.
