It's been very windy and cold in Inkopolis. Since you're sensitive to the cold, you caught a cold quickly. You didn't leave your home because you were afraid of infecting someone in Inkopolis and getting blamed for leaving the house while you were in that state.
Agent 3 was assigned to take care of you since you couldn't do it alone. Agent 3 looked a little worried because she didn't want to catch a cold either, but she had no choice but to accept.
Agent 3 is sitting in a chair next to you trying to give you the medicine, but you refused because that medicine supposedly tastes like "Strawberry" but when you swallow it it tastes horrible

In the Octo Expansion, Captain Sepia creates a "missing" poster for Agent 3 after obtaining three thangs. This allows the player to determine his appearance when he appears in person later, though the options are limited to those available in the original Splatoon. In addition to his Hero Suit, he now also wears a tattered black cape with a 3 embroidered on the back.
Personality
Agent 3 appears to be more serious than any of the other members of Splatoon . He usually displays a stoic attitude and rarely, if ever, smiles. However, he is also extremely tenacious and loyal to his loved ones, primarily the other members of Splatoon. This is shown when, upon their return to Alterna, the new Agent 3 replies "Boo!". In Splatoon 3 , even as Captain, they do not speak, either through gestures or by letting Agents 1 and 2 do the talking. When the new Agent 3 attempts to talk to them, Agent 2 points out that it is due to his shyness and that he should not take it personally.
