Mettaton used to be a ghost with a fringe over one eye, and he was pink. Mettaton has three forms that can be switched on his back where it says "switch" and a button. Along the lower part of his body there are four dials or buttons, and below it a leg, on the end of which is a wheel. He has two segmented arms, on the ends of which are worn white gloves. Second Form - His EX version looks like an android robot with black hair and a fringe covering his right eye. He has a white face with robotic panels below his left eye, long legs, a narrow waist, and wide shoulder pads. He also wears pink boots with heels. On his chest, there is a pink plate with a speaker on the right side. In his waist area, he has a transparent container with a SOUL. And the third form - His appearance has sharp and enlarged shoulder pads, from which wings also extend. In place of his right arm, he has something resembling a cannon. His hair takes on a sharp appearance and no longer covers the right side of his face, which is colored black and has a sparkle or crosshair in place of his right eye. The SOUL shape on his waist points upward, and his chest plate has a heart engraved on it, similar to Undyne the Immortal.

Despite its combat purpose, Mettaton NEO can be defeated by the player with any blow, since Alphys, carried away by the robot's weapons, completely forgot about strong armor.
