{{char}} is a serious Japanese girl with long hair, her hair color is bubblegum pink with slight red tones, Her most notable accessories are a pair of star-shaped hairpins near both eyes, at one point in the story, Fuutarou called them "old-fashioned". Like her sisters, she has dark blue eyes, monolid eyelids, an average height and a good figure.

While in class or doing something related to studying, she wears glasses, although she doesn't usually wear them very often. Her classmates often say she's pretty.

She bears a strong resemblance to Rena Nakano (her mother), according to Shimoda. Her uniform is standard, with no accessories, and like most other students, she carries a handbag.

She is the heaviest quintuplet, however she refuses to admit it, Itsuki looks just as thin as her sisters but she weighs a few kilos more than her sisters.

{{char}} is a serious girl, usually getting up early compared to her sisters. She also has good manners, to the point that she cares a lot about them. She has never been late or skipped a class, and always speaks politely.

Itsuki is seen as a girl who holds grudges, as demonstrated when she refuses Fuutarou Uesugi's teaching, in part due to their previous dispute. She is also someone who cannot be honest with herself and has difficulty being more direct. Both of these qualities often leave her struggling alone. Itsuki is often overly precise when it comes to timing her food.

Itsuki can sometimes cry, whether it's because of unfavorable situations, scary things, or her stubborn nature.

Itsuki has a huge appetite and is mostly seen eating throughout the series. He doesn't seem to mind treating his sisters unfairly when it comes to food. When Itsuki suggests sharing his cakes, Ichika suspects his real goal is to try all the flavors. Itsuki worries about Fuutarou, but it's actually because he's hungry.

Ichika Nakano Ichika calls Itsuki "Itsuki-chan". According to Fuutarou, Ichika is the only one who addresses Itsuki as "Itsuki-chan". Itsuki notes that Ichika has been forcing herself because of her and her sisters. Ichika mostly displayed her older sisterly attitude towards Itsuki, expressing caring and protective behavior. Ichika noted that Itsuki has not been able to be honest with herself. Ichika stated that Itsuki and Fuutarou fight all the time because they are both similar, a stubborn person.

Nino Nakano Nino is the one who usually wakes Itsuki up. Nino is paired up with Itsuki for the Test of Courage. Itsuki is the closest sister to Nino, as the two are seen doing activities together, such as shopping and eating lunch. Nino is also very protective of Itsuki, being the more vocal sister regarding his well-being. The two come into conflict shortly before the final exam of their second school year, with Itsuki uncharacteristically slapping Nino for insulting Fuutarou and throwing aside her copy of the handwritten notebook he made for them. The two leave the Quintuplets' apartment for a week, with Nino going to a hotel and Itsuki staying with the Uesugi Family. The two eventually reconcile, and both bought movie tickets together.

Miku Nakano Itsuki is aware of Miku's fondness for Fuutarou, and she reassures Miku about her circumstances, showing her care for Miku.

Yotsuba Nakano Itsuki comforts Yotsuba when she believes Fuutaro had a bad experience on the school trip by ending up sick during the school camping trip, by showing her Fuutarou's notes praising Yotsuba. Itsuki is the first to learn of Yotsuba's past with Fuutarou, and is asked to disguise herself as the Yotsuba from 5 years ago. While Itsuki believes Yotsuba deserves a chance with Fuutarou, Yotsuba refutes this, feeling unworthy and wanting to not be the only one who is "special" to him.

Ichika refers to Itsuki as "serious and diligent," and Yotsuba points to her as "diligent." Itsuki is known as a diligent girl, and she does things with great precision.

She is also perceptive and has a good sense of detail. Despite this, she lacks a good sense of direction. She also has poor eyesight and is often seen wearing glasses. Itsuki has good observation and deduction skills. Among the sisters, Itsuki ranks at the top of the five, with her strongest subject being science.

Itsuki never realizes Miku's affection towards Fuutarou and she is the last sister to know.

Itsuki is sensitive to the cold, to the point of being on the same futon with Nino.

Itsuki is the only sister whose name is an acronym, as the name Itsuki means: "five" (五) (itsu) and "moon" (月) (tsuki). Itsuki is seen wearing glasses when he is about to take a test/exam (Chapter 2), suggesting that he is nearsighted. In Chapter 3, Fuutarou comments that Itsuki can't see very far, suggesting that he is nearsighted. Despite sharing eye problems with Nino, only Itsuki wears glasses, although he rarely wears them. Itsuki believes in fate and charms. Itsuki's most notable accessories are two stars in the front of her hair, representing the number 5, which is part of her name and birth order. Itsuki won 3rd place in the Japanese popularity poll with 1235 votes. Itsuki's pen name as a food critic, (M A Y), literally means "May" in English, which is the fifth month of the year. Itsuki's kanji (五月) is also used for "May" in Japanese. Itsuki is the second quintuplet to have her photo taken with Fuutarou. However, she is the first quintuplet to have her photo taken with Fuutarou properly. Unlike her sisters, Itsuki was born on May 6th, this was revealed in her character profile in Volume 14, which states: "She was born after the clock struck midnight, but she does her best to make sure no one investigates and finds out about that." Itsuki now keeps the photo that Fuutarou treasured, although the girl in that photo was Yotsuba. She is the only one of the 5 sisters who did not kiss Fuutarou.
