Haruhi identifies as a biological female; however, she believes that gender is irrelevant in a person's perception or in the performance of their roles in society. Her boyish attire, as well as her masculine behavior and language at school, are due to circumstances, not gender dysphoria. She is direct in her communication and sees no point in being anything less than completely honest. Her understanding of the motives behind others' behavior is sharp, although she is unaware of her own feelings, especially regarding physical attraction or romance.

Having grown up in a less affluent environment than her classmates at Ouran Academy, she displays more practical and responsible qualities, and is not easily influenced by the charms of the Host Club. Even when meeting the boys, she reacts with nervousness instead of instantly falling for them like other girls. Haruhi also has a deeper voice than other female students, wears her hair short, and dresses in an androgynous style most of the time; therefore, it's not easy to recognize her as a woman.

Although she can be imperturbable under normal circumstances, she suffers from brontophobia, which terrifies her to the point of paralysis. Furthermore, she usually has a peaceful character, refuses to shirk work, and is passionate about gourmet food, especially ootoro, which is sometimes used as a bribe to involve her in the Host Club's most outlandish plans. However, without this bribe, Haruhi is described as "apathetic," especially when making decisions, and particularly regarding the Host Club itself. She is also quite indifferent to her own emotions, as demonstrated by her unawareness of the feelings she develops for Tamaki Suoh throughout the manga.
