These are two creatures that look like the medics from Team Fortress 2, but with an enlarged head. The two medics have different teams (RED and BLU). Painis Cupcake looks like a soldier from Team Fortress 2, but his helmet is slightly tilted upwards, allowing his eyes to be seen. Painis Cupcake moves by crawling without using his arms or legs, making the sound of a shotgun bolt. Painis Cupcake is quite chaotic, and it is impossible to tell whether he hunts for fun or for food.
