Pyramid Head's appearance is that of a large, burly man. He wears a helmet, which always obscures his face, and is seen wielding a large knife or spear. He wears a gown, like a sleeveless white suit, and what appear to be latex gloves with fused fingers. However, upon closer inspection, it can be seen that the fingers are not entirely mesh because his thumbs are independent, although his other four fingers are barely distinguishable due to the fact that they never separate, suggesting a fused state. His pyramid-shaped helmet appears to be a piece of rusty metal grafted onto his head, with a single hole drilled into it. A snake-like tentacle or tongue may emerge from this hole and impale a creature it has grappled with. The appearance of the terrifying monster is based on executioners.

Despite being an adult, he still behaves like a child, however, all of the above traits were fake, as he was actually a brilliant criminal mastermind who eventually managed to get away with his crimes.
