When Fraguo launches his attack on the Mushroom World, he decides to take over his castle. Thus Exor, the dimensional portal to Fraguo's world, crashes into the castle and occupies it. However, as Exor fell from the sky, he caused the Star Path to be destroyed. As a result, the seven stars are scattered across the Mushroom World. With the Star Path broken, wishes cannot be granted. Because of this, Geno is sent to retrieve the stars and restore the power of wishes.

Some time later, the town of Rosedal is raided by Fraguo's Gang. Arquelio, one of Fraguo's henchmen, is firing arrows into the small town that are capable of paralyzing them. As a result, people in the town of Rosedal are unable to leave their homes for fear of being paralyzed by one of Arquelio's arrows.

During this time of crisis, Mario and Mallow arrive in the town of Rosedale. Seeking shelter from the arrows, the pair enter the town's inn. Inside, Mario sees a young boy named Gaz playing with his Mario, Peach, and Bowser dolls. He is reenacting one of several Mario adventures. Gaz's mom, who is also the owner, asked the plumber to play with her son. Gaz pretends to be Mario, and Mario pretends to be Bowser. During the pretend battle, the Mario doll is defeated, so Gaz brings in his new Geno doll. However, Gaz plays a bit rough, activating "Super Duper Ultra Enhanced Star Attack" and instead of hitting the Bowser doll, the explosion inadvertently hits Mario, knocking him unconscious.

That night, being left with only one star, the one later known as Geno, enters the inn. Seeing the four dolls on the ground, the star decides to inhabit Geno's doll. Later, Geno admits that he chose that doll as it was apparently the strongest of the four.

The next morning, Gaz tells his mother that he saw Geno walking into the Wooded Maze. Because of this, his mother believes it to be a lie (which, of course, it isn't). Mario instead believes the boy's tale and enters the forest to find the doll. Mario eventually finds it in the nick of time to save it from Arquelio's arrow. Geno enters the forest to challenge Arquelio and retrieves the Star Piece that Arquelio discovers. Mario and Mallow decide to help Geno, and together, the three defeat Arquelio and acquire the Star Fragment. After defeating Arquelio, Geno realizes that he needs help to retrieve the remaining Stars and decides to join forces with Mario and Mallow. Geno reveals to the heroes that he is a warrior of the Star Way and that he takes the form of a doll to fight Fraguo's minions. He also states that until all the Star Fragments are retrieved, wishes cannot come true again. He also reveals that the star's name is "♥♪!?". However, this is impossible to pronounce, so the star warrior assumes the doll's name, Geno.

Eventually, Mario, Mallow, Geno, Bowser, and Peach collect the remaining pieces and defeat Fraguo. With Fraguo destroyed, Geno decides to abandon the doll's body, returning to the form of a star and taking the fragments with him. After the Star Path was repaired, the power of wishes returned to the world. The lifeless Geno doll can be seen sitting behind Mario and company as they witness Exor disintegrate.

Geno acts as a wise/sensible figure to Mario and his allies. He has an understanding of the bigger picture and the greater consequences of cosmic events. As a result, he explains to the heroes about matters that the others cannot understand. For example, after Mario and his allies arrive at Bowser's Castle, Bowser decides to leave the team and return to the castle. However, Geno points out that, until Forge and its factory are destroyed, Bowser's Fortress is still at risk. He has a variety of attacks that utilize a mix of physical and energy projectiles. His regular attack involves firing physical projectiles from his arms. His special attacks typically involve powerful energy blasts. Additionally, he is the fastest of Mario's allies.

Geno's personality is heavily based on confidence and attitude. His eagerness to confront the villain may be due to his status as an authority figure of the Starway.

He also tends to show more enthusiasm than any other member of the party, perhaps second only to Mario, but it's hard to say that Mario is a silent protagonist throughout the game.

He appears to be very strong from his first appearance as a doll when Gaz attacks using the Geno doll and accidentally hits Mario and knocks him out. However, when he tries to fight Arquelio, upon being hit by Geno he will say "You are strong, but I am stronger!". One of Geno's magical powers is also potentially the most powerful attack in the game, 'Geno Whirl'. If timed just right the Y button is pressed it will cause 9,999 points of damage.

Although powerful, Geno is quite kind. He has a strong sense of duty and responsibility and his role in the Starway can be analogous to Mario's role in the Mushroom Kingdom.

Despite his strength, Geno does not have much HP unlike Mario, although his speed surpasses that of the plumber, and his strength is superior to Bowser, making Geno good at taking down difficult enemies. When equipped with the feather, Geno is the only character capable of defeating a Shy Range. Most of his magic is difficult to execute perfectly.

Attacks
Geno Beam: He knows it from the start. Hold down "Y" to make Geno charge his power and release a blast that hurts the enemy, spending 3 FP and MP is 40.
Geno Boost: You get it when you reach level 8. Geno increases the attack power of the selected character, if you press "Y" right after the arrows disappear, his defense will also increase, he spends 4 FP and his MP is 0.
Geno Whirl: You get this when you reach level 11. Geno throws out a kind of cutting wind discs, if you press "Y" just before hitting the enemy to instantly kill them, although it is much harder to do (this does not apply to bosses, except Exor or Yaridovich copies), it costs 8 FP and its MP is 45.
Geno Blast: You get it when you reach level 14. Press "Y" to charge this attack in which Geno launches a small beam into the sky falling in a large variety of lights that hurt all enemies, spends 12 FP and its HP is 50.
Geno Flash: You get it when you reach level 17. Geno becomes a kind of cannon that launches a small ball of energy that causes a lot of damage. If you press "Y" before the impact, the damage will be increased, it costs 16 FP and its MP is 60.

His appearance design could be a parody of Pinocchio.
Along with Mallow, he never appears in any other video game since they are owned by Square.
His name Geno is a nickname, since his real name is unpronounceable (♥♪!?).
Despite having appeared in only one installment, Geno is one of the most famous characters.
He was also supposed to have an appearance in the video game Super Smash Bros. Brawl, which would not have been possible due to rights and lack of agreements between Nintendo and Square, since as mentioned, Geno is a character that belongs to Square.[citation needed]
In the video game Super Smash Bros. for Nintendo 3DS and Wii U, Geno appears as a 'Mii Shooter' costume.
