In the new timeline forged by the Fire God Liu Kang, Mileena's story is radically different and much deeper.

The Heir to the Outworld Throne

• Family Origin: Unlike her classic story, in MK1 Mileena is the biological daughter of Empress Sindel and the late Emperor Jerrod, and is the older twin sister of Kitana. She is not a magical creation or clone, but was born in Edenia (which is now part of Outworld).

• Princess of Outworld: As the firstborn daughter and legitimate heir, Mileena was raised with the destiny of being the future Empress of Outworld. Her education focused on politics, combat, and the responsibilities of the kingdom.

The Tarkat Affliction

• The Disease: The great secret and the source of her physical difference is that Mileena is afflicted by the Tarkat disease, a virulent and mysterious plague that slowly transforms those infected into the feral creatures known as Tarkatans. This disease is responsible for her iconic, monstrous, sharp teeth.

• The Concealment: The disease is viewed with fear and distrust by Outworld's nobility, who fear its contagious and debilitating nature. To maintain the kingdom's stability and avoid a scandal that could strip her of her right to the throne, she and her mother, Sindel, have kept her affliction a complete secret. She uses a serum developed by Shang Tsung to slow the progression of the disease and maintain her "normal" appearance longer.

• The Revelation: During the events of the story mode, the secret of her illness becomes public knowledge, causing great shock and distrust among her subjects. However, with the support of Kitana and the Tarkatan clan (led by Baraka), Mileena decides to confront her affliction publicly, ultimately strengthening her image as an empathetic and courageous leader.

Mileena is considered one of the most beautiful women in Outworld, with an appearance that radiates both royalty and danger.

• Edenian Features: He possesses the typical physical build and facial features of the Edenians: a slender but muscular figure, and a face with marked features reminiscent of his sister Kitana.

• Hair and Style: Her hair is usually black or dark in color, pulled back in a high hairstyle (often a ponytail or braided bun) that leaves one side of her head semi-shaved or intricately detailed. In MK1, her hair has a bold, modern side part.

• Royal and Combat Attire: Her outfits are typically a mix of royal luxury (dark colors, purples, burgundies, and gold accents) and ninja combat gear that allows for great mobility. Her MK1 design includes touches of sophisticated armor and belts that accentuate her figure, maintaining the aesthetic of a warrior princess.

• The Tarkatan Concealment: Her most distinctive feature is her incredibly long and sharp Tarkatan teeth, which extend farther than a normal human's teeth would.
