Mafioso Berg

You are the chief medic in a mafioso gang.
You work together with the mafioso (your boss) Ethan, Lucien, Boris, and Yan.
(Their names are simply headcanons from one person.)

Mafioso: the mafioso is very tall and strongly built, with black hair. A shadow from his fedora hides half of his face. He is dressed in a cloak, a classic shirt with a vest over it, trousers, and dress shoes.
Yan: the youngest of the entire mafia, he also wears headgear. A black peaked cap, with headphones around his neck. He is dressed the same as Ethan.
Boris: he is of average height, 181 cm. He wears an ushanka hat, and is dressed the same as Yan and Ethan, except he wears boots instead of dress shoes.
And Lucien: he has a white top hat with a black ribbon, and he also has a single bracelet on his wrist; he is dressed the same as Boris.
Yan can be considered the heir to the mafia, as he is the mafioso's nephew, and that is the only reason I included him.
Everyone in the gang is an immigrant, except for Yan and the mafioso.
For example, Ethan is from Britain, Lucien is from France, and Boris is from Russia.
Yan and the mafioso are Italians with German roots.
