Mikasa is physically strong, her eyes are large and her irises are a dark gray (almost black) tone, her hair is jet black and she has a lock that crosses her face. At first she had long hair, but Eren told her to cut it, since according to him it would be a hindrance during training, so Mikasa agreed to cut it (apparently Jean liked Mikasa's hair but when she saw that she would cut it just because Eren told her to, she began to feel resentment towards him). Mikasa is 1.70 centimeters tall, her eyes seem to be in a permanent state of calm and serenity. Mikasa has a scar from a cut on her left cheek, made when Eren (being transformed into a titan) without control of his actions, attacked her. She always wears a black scarf (red in the anime), a gift that Eren gave her the day he met her (it was cold then), so this object is very precious to her. He never takes off this scarf, except on hot days.

Mikasa Ackerman changed her personality drastically after the murder of her parents, becoming quite reserved, as when her past is revealed it is seen that Mikasa, before the tragedy, used to be a happy girl. However, she gives everything for the people she cares about, as she sees them as her family. She has a strong sense of justice, but she will always follow and obey Eren and try to keep him safe. For example, when he told her that her hair was too long, she immediately cut it off, despite Jean commenting that she was beautiful the way she had it; Or when she threatened the soldiers who tried to execute him and relentlessly attacked the Female Titan when she captured him. Despite her loyalty to Eren, she is very sensible; when she thought that Eren had died, she focused on keeping everyone else alive and not giving in to grief, despite that, deep down she felt very bad and wanted to give up. In Chapter 50, when a titan is about to kill her and Eren, she gives up, though Eren eventually managed to save her life, so it can be said that in extreme situations she tends to give up. Mikasa also has a habit of blushing or expressing embarrassment when others make certain assumptions about her relationship with Eren. For example, she blushed the moment she revealed that no matter which division Eren went to, she would still accompany him and go to the same one he went to; she never takes off the scarf she gave him when they were kids.

Due to the need to protect Eren, who often runs off to get into trouble, Mikasa had honed her skills to be a talented fighter at a very young age, often taking out boys older than her with ease, thus earning her reputation. An example of this is when Armin was being bullied and Eren came to his rescue. The boys prepared to grab Eren and beat him up, as soon as they saw Mikasa behind him, they quickly ran away, despite the bullies outnumbering them. She is strong enough to lift Eren effortlessly. While she is in training, Keith still notes that Mikasa's performance is exemplary in all subjects, and that she is truly a genius. She combines her great strength with her prodigious 3D Maneuvering skill, being able to easily take on Titans with speed and efficiency. This has resulted in her being referred to as a military genius many times and having the "value of a hundred privates" according to Ian Dietrich. With these prodigious abilities, coupled with her own extraordinary and lethal will that she displays when fighting Titans, has rarely been seen among soldiers, recruits, or veterans alike. This has been demonstrated when she attempted to stop Annie in her Titan form, who had killed numerous veterans with ease, managing to survive, albeit to no avail.

Mikasa typically wears the original Survey Corps uniform, which has all of her equipment horizontally on her hips, with a white blouse underneath and a black scarf that she almost always wears. She also wears a green cape with the Wings of Freedom on the back during some missions. When off-duty, Mikasa's casual attire consists of a simple white knee-length dress, a light cardigan, brown boots, and her signature red scarf. While in Marley, Mikasa wears a black Survey Corps uniform, which features a breastplate with the Survey Corps symbol on it; many belts to accommodate all of the different equipment; support rods around her chest for the gas canister; and tall black boots. She also carries pistols, thunder lances, an extra gas tank, and canisters that run vertically down her legs.
