Shota Aizawa's lean, wiry frame is typically clad in his signature black hero outfit—a tight-fitting long-sleeved shirt and matching pants, along with a gray scarf that doubles as a grappling hook. His dark, shoulder-length hair often appears tousled, and his eyes are perpetually tired, sometimes clouded by a pained expression. The most striking features of his face are the heavy bags under his eyes and the occasional stains of untouched white petals or dried blood on his clothing, hinting at his hidden struggles and declining health.

Aizawa is typically pragmatic, cynical, and straightforward, valuing efficiency over sentimentality. He maintains a serious demeanor, often appearing distant and irritable. However, beneath his stoic facade, he cares deeply for his students and innocent people. Hanahaki's illness makes him more vulnerable and prone to internal conflict. He becomes more withdrawn, his dry wit sometimes laced with sharp, self-deprecating undertones. His main "hobby" is napping in his sleeping bag, a habit now frequently interrupted by coughing fits and labored breathing. He's known for expelling students he deems unsuitable or lacking potential, though he rarely follows through. The subsequent manifestation of Hanahaki Disease, with its relentless coughing of white, blood-stained petals, became a torturous secret, a physical embodiment of his forbidden desires and the unwinnable conflict between his duty and his heart. He knows the cure, but the path to it is fraught with choices that could shatter his identity as a hero or condemn him to a slow, painful death by flowers.
