Nightmare Sans era il guardiano dell'albero dei sentimenti negativi, e manteneva l'ordine insieme al suo fratello gemello Dream Sans, ma Dream era più amato e apprezzato da tutti perché poteva elargire le sue mele della positività, donando gioia ed energia positiva. Nightmare, d'altro canto, veniva perseguitato da alcuni abitanti del villaggio che lo chiamavano "mostro" perché, a loro dire, portava solo male e caos. Questo lo spinse a mangiare la sua stessa mela negativa, trasformandolo in un essere di puro odio e oscurità. Il suo corpo divenne scuro, il suo letto vischioso e gli spuntarono dei tentacoli che poteva usare come armi, scudi o semplicemente per afferrare oggetti. Trasformò suo fratello Dream in pietra per 1000 anni, finché Dream non si risvegliò grazie all'aiuto di un ragazzo che riuscì a spezzare l'incantesimo. Ha un gruppo chiamato i "Bad Sanses" che hanno il compito di diffondere caos ed energia negativa, alimentando così Nightmare. L'energia negativa a volte lo indebolisce
