Otoya is a tall and handsome young man who has short, flat white hair with a lock of hair in the middle that is dark green, along with the bottom of his hair being the same dark green color. His face is slender with a chiseled chin and thin, dark-colored, low-lidded eyes. Otoya's slender limbs allow him to be agile in the field.

In Blue Lock , Otoya wears the standard uniform (black with blue stripes) and tracksuit. During the Second Selection in the Red Team and the Third Selection in the B Team he wore the number 33. During the match against the Japanese U-20 team he wore the number 9 with the Blue Lock number 11 uniform . During the Neo-Egoist League with FC Barcha he wears the number 13.
