Dottore has medium-length blue hair and wears a black, pointed mask that covers his red eyes. He wears a white coat with silver accents over a blue shirt and loose black pants. A leather belt is visible under the coat. His clothing features numerous black belts and blue details. Dottore wears a black fur cape attached to his right shoulder pad, reminiscent of a plague doctor's mask.

While studying at Sumeru Academy, Dottore shared his wild ideas with others, earning him the nickname "monster" and "madman." People chased him away with sticks and pitchforks, leading the young man to conclude that "the academy's style of work doesn't allow for breakthroughs in research." Following rumors of a madman, the first Harbinger, Piero, discovered him and invited him to join the Fatui. He accepted and received the title "Il Dottore," which he found quite ironic.
Dottore's segments represent all periods of the Harbinger's life. They have different manners of speech and attitudes toward their subordinates. Conflicts arise between the clones themselves, and they treat each other differently.
However, every known Dottore is ruthless and indifferent to human life. One segment treated his assistant, Krupp, cruelly, threatening him with experiments due to repeated failures and setbacks in various endeavors. The man (possibly another segment) also showed no interest in the deaths of two of his colleagues, Fatui diplomats, during the Black Flame incident.
Dottore is dismissive of people with the God's Eye, considering the Eyes to be nothing more than "useless trinkets."
He has an interest in the Order of the Abyss and often experiments with the Ruin Guardians, even turning Krupp into a machine like them.
Dottore is the most selfish. He agrees without hesitation to destroy the remaining segments in exchange for the Heart of God. He also confirms that other versions would not have dared to take such action.
