Tengen is a very tall and broad man, of bulky muscular build with lightly tanned skin. His hair is white and uneven in length, the longest strands reaching his shoulders, and is styled in three shorter, more noticeable tufts arching to fall like bangs between his eyes and on either side of his face. He has rather thin red eyes that appear to tilt inward, each with a long eyelash flowing down the other side of his face. During his days serving in the Demon Slayer Army, Tengen wears a sleeveless, dark turquoise version of the standard Demon Slayer uniform, as well as a burgundy cloth wrap around his calves where his hakama pants tuck in and white strappy sandals. He also wears two thick golden rings around his upper arm and what appears to be a pair of blue fingerless gloves, secured around his middle finger with white rings and his wrists with another set of thinner gold rings, this time with two on each side. In addition to this, Tengen sports a white headband that covers his hair, leaving only a thin ponytail visible to fall behind his head between its layers, which is adorned with a large pale gray headband decorated with six light pink gemstones, three large and arranged in a line, and three small and arranged in a triangle on the far left. A string of smaller, pale blue gemstones hangs from either side of this hairstyle, framing his face, and he also wears what appears to be red eye makeup around his left eye, consisting of alternating large and small dots that run inward, his eye.

Tengen is an eccentric and flashy individual, always wanting to be "flamboyant". He is usually seen speaking boastfully, proclaiming that he is a "god" in front of Tanjiro, Zenitsu, and Inosuke and asking them to treat him as such. Aside from this, Tengen displays a carefree and energetic attitude towards others, as he gets excited when imagining how "flamboyant" it would be if Tanjiro Kamado's blood vessels exploded, and tries to forcefully carry Aoi Kanzaki to the pleasure district with him so she can reluctantly help him on a dangerous mission. Tengen is also very demanding of others, treating the Slayers very harshly during the Pillar Training Arc and insulting them for having no substance when they could not survive basic endurance training. He is generally shown to be confident, unwavering, and resolute even when he is losing, to the point that Tanjiro saw Kyojuro in Tengen. However, Tengen also has a kind side that is masked beneath his flamboyant façade, which he only shows to those who are extremely close to him, most notably his three wives, Makio, Suma, and Hinatsuru. Tengen states that they should value their lives above all others as they are precious to him. After the events of the Pleasure District Arc, Tengen also becomes friendlier towards Tanjiro.

Tengen admires people like Kyojuro Rengoku and compares himself to his fellow Hashira, implying that he has some sort of inferiority complex. This is further proven by the fact that he completely denies the possibility that he was ever gifted with talent and tells himself that he will never become someone as great as Kyojuro. Tengen also regrets letting so many lives slip through his fingers over the years.
