He should be 43. By the end of Season 4, he's shown still trudging through Purgatory, a place where time progresses more rapidly than it does in the Living World, meaning that by the time he escapes, he'll likely be thousands of years older than 43, but due to his immortality, won't possess a wrinkle to show for it.
Has anyone even noticed how grave the situation he is in is? As stated earlier, 1 minute on earth is roughly a year in purgatory. It has been over half a DAY since he entered purgatory. Half a day is about 720 years in purgatory.
