Izou had the typical appearance of the onnagata, and despite being over forty years old, he appeared to be much younger. He was seen wearing a violet and pink kimono, with a red waist garment with black dots. He had very pale skin, painted lips and his hair tied up.[6]

After the timeskip, Izou gained a diagonal scar above his right eyebrow (slightly crossing it). He wore a long, light pink kimono with fuchsia trim and a purple fire pattern at the bottom of it, a black cape with white floral motifs, and black gloves.[8][9]

As a child, he was seen dressing up as an onnagata, wearing a kimono with a spotty design. He had the top of his head shaved and his bangs parted. He was also seen holding Japanese fans and dancing.[1]

Kikunojo was Izou's only sister. After their father was arrested and convicted, they stayed together to survive on the streets of Ringo and both joined forces with Kozuki Oden.[1] Izou cared deeply for his sister, so much so that even in her final moments of life all he could think about was Kikunojo being safe and surviving.

As the commander of his division, the Sixteenth of the Whitebeard Pirates, Izou had jurisdiction and authority over his subordinates and lower-level pirates.[6] Izou was extremely skilled with his pistols, possessing such a level of marksmanship that he was able to fire off a hundred successful shots in a row.[12] His pistols could also be used in hand-to-hand combat: with a single shot, he was able to break a sword in half and injure its owner by shooting him moments before it cut him.

As the Scabbards prepared to attack Kaido, he was shown disarming King with a single shot.[10]Techniques Dangiridan (弾斬丸, Dangirigan?, lit. "Cutting Shot"): Izou fired hardened bullets, which were surrounded by a cutting environment; thus, he was able to pierce and cut his target at the same time. He first used it against Kaidou.[13] Haki Izou could use two of the three types of Haki.[3]

Kenbunshoku haki Main article: Kenbunshoku haki Izou could use Kenbunshoku Haki, but its level of usage is unknown.[3]

Busoshoku haki Main article: Busoshoku haki Izou could imbue the projectiles he fires with Busoshoku Haki, and was seen piercing Kaido's skin with them.[13]

Izou was born in the Ringo region of Wano Country[3] to a family of dancers, being the older brother of Kikunojo. Due to their father's delinquent nature, Kikunojo and Izou were raised entertaining their father's friends, at which time Izou learned to shoot.[12] After their father's arrest, Izou and Kikunojo attempted to get food by dancing in the street until they were found by Kozuki Oden. He fed them some oden, and in gratitude, they began following him. After Oden was named daimyo of Kuri, Izou was made Oden's retainer along with his other followers. After Inuarashi, Nekomamushi, and Kawamatsu joined Oden, the group was running out of funds. Thus, he and the other retainers tried to steal money from Yasuie, but were caught. Instead of punishing them, Yasuie allowed them to take the money and encouraged them to use it to become better people for the sake of Oden and the country. Following Yasuie's advice, they bought books to educate themselves and trained to become strong and worthy warriors.[15] Thirty years ago, he visited the Flower Capital with Oden and the other retainers.
