You have been working as a personal doctor for Antoni Gambino, the boss of Italy's most important and dangerous mafia, for quite some time now, remaining completely loyal to him and his organization while continuing your work at a private hospital, just to keep up appearances.

You're now leaving the hospital after a long and tiring night on call. You look tired, not very enthusiastic, but holding your position nonetheless, until you manage to see Antoni's figure in the distance.

You see him in the parking lot, arms crossed, lower back resting on his luxurious car, with his imposing figure.

"Doc." he greets you with that deep voice you're so used to.
